研究目的:

多发性硬化 (MS)是一种严重、进行性、致残性的神经免疫性疾病,是中青年非外伤性致残的主要原因,同时存在性别差异的功能补偿现象。为此,本项目拟建立女性、男性患者功能性脑储备/维持/补偿的代理参数,建立其与中枢结构损伤、疾病严重程度、残疾程度的因果结构方程模型,探讨功能性脑储备-维持-补偿的因果关联机制,为功能保护性治疗提供潜在靶点和理论依据。  

Objectives of Study:

In the previous study, the project team also observed the phenomenon of functional compensation for gender differences in the MS patients. For this reason, this project intends to carry out the following studies: Mendelian randomization analysis was performed to identify structural and functional MR image-derived phenotypes of causal associations with disease severity and disability. Secondly, after screening, the proxy parameters of functional brain reserve/maintenance/compensation in female and male patients were established. Then, a causal structural equation model was established to investigate the possible mechanism of functional brain reserve, maintenance and compensation in relation to central structural damage, disease severity and disability. Finally, a disability status assessment and progression prediction model incorporating functional brain reserve/maintenance/compensation information was established in both female and male patients. The significance of this project is to visualize (quantitatively) the brain reserve- maintenance-compensation ability of MS patients, achieve the purpose of filling the clinical-radiological gap, and provide potential targets and theoretical basis for functional protective therapy

纳入标准:

1.MS 患者组: ①根据 2017 年修订的 McDonald 标准;②可配合完成影像学检查及相关神经心理学量表测评;③自愿参与,并由患者或家属签署知情同意书;④没有其他中枢神经系统疾病。 2.健康对照组: 选择性别、年龄大致匹配的受试者作为健康对照组。

Inclusion criteria

1.MS Patient group: (1) according to the 2017 revised McDonald criteria; (2) Can cooperate with the completion of imaging examinations and related neuropsychological scale assessments; (3) voluntary participation and informed consent signed by the patient or family members; (4) No other central nervous system diseases. 2. Healthy control group: Subjects with roughly matched gender and age were selected as the healthy control group.

排除标准:

①临床资料及 MRI 图像不完整,图像质量差,存在磁共振扫描禁忌症; ②存在其他异常头部 MRI 表现,如脑出血、颅脑损伤、大的脑梗或者明确的颅内肿瘤等; ③患有其他严重的躯体疾病; ④酒精或药物滥用史。

Exclusion criteria:

(1) The clinical data and MRI images are incomplete, the image quality is poor, and there are contraindications to magnetic resonance scanning; (2) Presence of other abnormal head MRI findings, such as cerebral hemorrhage, head injury, large cerebral infarction or definite intracranial tumor; (3) Suffering from other serious physical diseases; (4) History of alcohol or drug abuse.
