通过使用研发的体位枕更精准的进行头颈部制动，降低肩颈不适症状和术后并反症的发生率。  

Objectives of Study：

Through the use of the developed postural pillow for more accurate head and neck braking, reduce the incidence of shoulder and neck discomfort and postoperative complications.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

组织病理学诊断为头颈恶性肿瘤且需行游离皮瓣移植修复的患者；意识清楚，能配合完成调查；患者签署知情同意，自愿参与本研究。

Inclusion criteria

Patients with histopathological diagnosis of head and neck malignant tumor and need to be repaired by free skin flap; clear consciousness and can cooperate with the completion of the investigation; patients signed informed consent and voluntarily participated in this study.

排除标准：

术后有严重并发症；既往有认知功能障碍；肩颈部既往外伤史无法穴位按摩；对智能多功能中药枕过敏者。

Exclusion criteria：

There were serious postoperative complications, previous cognitive impairment and previous history of external injuries of previous trauma to the shoulder and neck that cannot be massaged; Those who are allergic to intelligent multi-functional Chinese medicine pillows.

干预措施：

常规围手术期护理和沙袋头颈部制动。操作方法：术前：（1）生活指导：①为患者制定科学、合理的作息规律；②指导患者调整膳食结构，均衡营养；③指导患者定时、遵照医嘱用药；（2）口腔准备：指导患者戒烟、戒酒，进食后漱口刷牙；（3）呼吸道准备：术前指导患者开展呼吸功能和咳嗽排痰的相关训练；（4）心理护理：讲解疾病相关的知识、手术流程和方法。术后：（1）气道护理：讲解气道护理的重要性和方法，出现突发事件的处理办法；（2）皮瓣护理：讲解头颈部制动的重要性，讲解血管危象的临床表现，术后立即给予沙袋头颈部制动3~5天；（3）营养护理：给予低脂肪、高热量、高蛋白、高维生素、营养丰富、易消化、易吸收的流质饮食并指导患者鼻饲的方法和注意事项；（4）心理护理：给予患者心理护理，缓解患者的心理压力，降低负性情绪；（5）健康教育：指导患者学习疾病护理和康复相关知识；（6）出院指导：指导患者加强营养、气管套管的维护以及皮瓣护理的方法。

Intervention：

Routine perioperative nursing and sandbag head and neck immobilization. Methods of operation: (1) Life guidance: (1) make scientific and reasonable routine for patients; (2) guide patients to adjust dietary structure and balanced nutrition; (3) guide patients to use medicine regularly and in accordance with doctor's advice; (2) oral preparation: guide patients to quit smoking and alcohol, gargle and brush their teeth after eating; (3) respiratory preparation: guide patients to carry out training on respiratory function and cough and expectoration before operation. (4) Psychological nursing: explain the disease-related knowledge, surgical procedures and methods. After operation: (1) Airway nursing: explain the importance and methods of airway nursing and how to deal with emergencies; (2) skin flap nursing: explain the importance of head and neck immobilization and the clinical manifestations of vascular crisis. Sandbag head and neck immobilization for 3 days immediately after operation (3) nutritional nursing: giving low-fat, high-calorie, high-protein, high-vitamin, nutritious, digestible, easy-to-absorb liquid diet and guiding patients' methods and points for attention in nasal feeding; (4) psychological nursing: giving patients psychological nursing, relieving patients' psychological pressure and reducing negative emotions; (5) health education: guiding patients to learn disease nursing and rehabilitation related knowledge. (6) discharge guidance: guide patients to strengthen nutrition, maintenance of tracheal cannula and skin flap nursing.

常规围手术期护理和体位枕管理。操作方法：在常规围手术期护理的基础上，增加体位枕应用。研究开始前由课题负责人对课题组成员进行培训，使成员全部掌握体位枕的使用方法。入院当天由课题组成员为患者详细介绍体位枕的使用方法，并让患者开始使用体位枕；患者手术毕返回病房后继续使用体位枕。

Routine perioperative nursing and postural pillow management. Operation method: on the basis of routine perioperative nursing, increase the application of postural pillow. Before the beginning of the study, the person in charge of the project will train the members of the research group to make them master the use of postural pillows. On the day of admission, the members of the research group introduced in detail the use of postural pillows for patients, and asked patients to start using postural pillows; patients continued to use postural pillows after returning to the ward after operation.
