研究目的：

1）筛选对胆囊结石或胆囊息肉形成血清胆汁酸谱和血脂代谢的敏感指标； 2）依据胆囊结石胆囊息肉复发风险分级给予随访行NOTES保胆治疗患者规范化的熊去氧胆汁酸预防性治疗； 3）构建胆囊结石胆囊息肉复发风险预警新方案。  

Objectives of Study：

1) Screening sensitive indicators for the formation of gallstones or gallbladder polyps, serum bile acid profile and blood lipid metabolism; 2) according to the risk classification of gallstone gallbladder polyp recurrence, the follow-up patients undergoing NOTES gallbladder-sparing therapy were given standardized ursodeoxybile acid prophylaxis; 3) Construct a new early warning program for the recurrence risk of gallstones and gallbladder polyps.

药物成份或治疗方案详述：

1）比较健康志愿者与胆囊结石或胆囊息肉患者的血脂代谢数据，检测并比较健康志愿者与胆囊结石或胆囊息肉患者的血清胆汁酸谱，筛选对胆囊结石或胆囊息肉发生敏感的血清胆汁酸谱和血脂代谢指标； 2）门诊随访已行经自然腔道内镜保治疗的患者，根据术后随访时间，是否有胆囊结石或胆囊息肉残留或复发，是否有胆囊疾病临床症状分为复发风险1级~4级患者。复发风险1级和2级患者，给予熊去氧胆汁酸预防性治疗半年；复发风险3级和4级，给予熊去氧胆汁酸预防性治疗一年。治疗前和治疗后分别对患者进行血生化血常规、血清胆汁酸谱检测和腹部彩超检查，分析治疗对患者血脂和血清胆汁酸谱敏感指标的影响，以及对胆囊结石或胆囊息肉复发风险水平和复发率的影响； 3）探讨基于胆囊结石胆囊息肉复发风险等级与胆汁酸谱和血脂代谢的敏感指标关系构建复发风险预警方案。 

Description for medicine or protocol of treatment in detail：

1) Compare the blood lipid metabolism data of healthy volunteers with patients with gallstones or gallbladder polyps, detect and compare the serum bile acid profiles of healthy volunteers with gallstones or gallbladder polyps, and screen the serum bile acid profiles and blood lipid metabolism indexes that are sensitive to gallstones or gallbladder polyps; 2) Patients who have undergone endoscopic treatment through the natural orifice are classified as patients with recurrence risk of grade 1~4 according to the postoperative follow-up time, whether there are gallstones or gallbladder polyps remaining or recurring, and whether there are clinical symptoms of gallbladder disease. Patients with grade 1 and 2 recurrence risk were given ursodeoxybile acid prophylaxis for half a year; Risk of recurrence grade 3 and 4, ursodeoxybile acid prophylaxis is given for one year. Before and after treatment, the patients were subjected to blood biochemistry routine, serum bile acid profile and abdominal color ultrasound to analyze the effects of treatment on the sensitive indexes of blood lipids and serum bile acid profile, as well as the risk level and recurrence rate of gallstones or gallbladder polyps. 3) To explore the relationship between the risk level of gallbladder polyp recurrence and the sensitive indicators of bile acid profile and blood lipid metabolism in gallbladder stones, and to construct an early warning scheme for recurrence risk. 

纳入标准：

健康志愿者： 1）体检志愿者、无疾病症状； 2）腹部彩超结果提示肝胆胰脾无异常； 3）无其他严重疾病； 4）入组时采集血清和粪便标本并冻存于-80℃。 拟行NOTES保胆治疗住院患者： 1）术前一周内有血生化全套检验； 2）术前三个月内有腹部彩超检查，提示胆囊结石或胆囊息肉； 3）行NOTES保胆治疗术后明确病灶为胆囊结石或胆囊息肉； 4）入组时采集血清和粪便标本并冻存于-80℃。 已行NOTES保胆治疗随访患者： 1）术前三个月内有腹部彩超检查，提示胆囊结石或胆囊息肉, 且术后明确病灶为胆囊结石或胆囊息肉； 2）同意并配合根据复发风险等级给予相应半年或一年的熊去氧胆酸治疗； 3）同意并配合治疗前和治疗后进行血常规血生化检验，腹部彩超检查，并给予留取血标本和粪便标本。

Inclusion criteria

Healthy Volunteers: 1) Volunteers with physical examination and no symptoms of disease; 2) The results of abdominal ultrasound showed that there was no abnormality in the hepatobiliary, pancreatic and spleen; 3) No other serious diseases; 4) Serum and fecal samples were collected at the time of enrollment and frozen at -80°C. Inpatients to be treated with NOTES cholecysticide: 1) A full set of blood biochemistry tests within one week before surgery; 2) Abdominal color ultrasound examination within three months before surgery, indicating gallstones or gallbladder polyps; 3) After NOTES chole-sparing therapy, it was determined that the lesion was gallstone or gallbladder polyp; 4) Serum and fecal samples were collected at the time of enrollment and frozen at -80°C. Follow-up patients who have undergone NOTES gallbladder-sparing therapy: 1) Abdominal ultrasound examination within three months before surgery, indicating gallstones or gallbladder polyps, and the postoperative lesions were confirmed to be gallstones or gallbladder polyps; 2) Agree and cooperate with the corresponding half a year or one year of ursodeoxycholic acid treatment according to the risk level of recurrence; 3) Agree and cooperate with the routine blood biochemical test before and after treatment, abdominal color ultrasound examination, and give blood samples and stool samples.

排除标准：

健康对照： 1）有明显疾病症状； 2）腹部彩超结果缺乏或提示肝胆胰脾异常； 3）有严重疾病，如长期高血压，糖尿病，肝硬化，癌症等； 4) 入组时未采集血清和粪便标本。 拟行NOTES保胆治疗住院患者： 1）术前一周内无血生化全套检验报告； 2）术前三个月内无腹部彩超检查； 3）术后明确病灶非胆囊结石或非胆囊息肉； 4）入组时未采集血清和粪便标本。 已行NOTES保胆治疗随访患者： 1）术后明确病灶为非胆囊结石和非胆囊息肉； 2）不同意或不配合根据胆囊结石或息肉复发风险等级给予相应半年或一年的熊去氧胆酸治疗； 3）不同意或不配合治疗前和治疗后进行血常规血生化检验，腹部彩超检查，以及给予留取血标本，粪便标本。

Exclusion criteria：

Healthy Controls: 1) Have obvious symptoms of disease; 2) Lack of abdominal color ultrasound results or suggestive of hepatobiliary, pancreatic and splenic abnormalities; 3) Have serious diseases, such as long-term hypertension, diabetes, liver cirrhosis, cancer, etc.; 4) No serum and stool samples were collected at the time of enrollment. Inpatients to be treated with NOTES cholecysticide: 1) No complete set of blood biochemical test reports within one week before surgery; 2) No abdominal ultrasound examination within three months before surgery; 3) Postoperative lesions are not gallstones or non-gallbladder polyps; 4) No serum and stool samples were collected at the time of enrollment. Follow-up patients who have undergone NOTES gallbladder-sparing therapy: 1) After surgery, it was clear that the lesions were non-gallstones and non-gallbladder polyps; 2) Unwilling or uncooperative to give ursodeoxycholic acid treatment for half a year or one year according to the risk level of gallstone or polyp recurrence; 3) Do not agree or cooperate with the pre-treatment and post-treatment routine blood biochemical tests, abdominal color ultrasound examination, and give blood specimens and stool specimens.
