研究目的：

本研究通过构建专科护士主导的老年癌痛患者出院准备服务方案，通过整合以患者为中心的可调动的所有资源，提供不同诊疗阶段的照护计划，保证患者得到持续而完整的医疗照护服务。  

Objectives of Study：

This study constructs a discharge preparation service program for elderly cancer patients with pain, led by specialist nurses. By integrating all mobilizable resources centered on the patient, it provides care plans for different stages of diagnosis and treatment, ensuring that patients receive continuous and comprehensive medical care services.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.通过临床诊断、病理分析以及影像学检查确诊为癌性疼痛；
2.年龄≥60岁；
3.服用吗啡60mg或同等剂量镇痛药物一周以上患者；
4.意识清楚，能理解调查内容；
5.患者及家属均知情同意，自愿参加本研究；

Inclusion criteria

1.The diagnosis of cancer pain is confirmed through clinical diagnosis, pathological analysis, and imaging examinations; 2.Age >=60 years old; 3.Patients who have been taking morphine 60mg or an equivalent dose of analgesic medication for more than one week; 4.The patient is conscious and can understand the content of the survey; 5.Both the patient and their family are informed and consent to voluntarily participate in this study.

排除标准：

1.合并严重精神疾患；
2.认知严重受损；
3.病情恶化，无法继续研究者；

Exclusion criteria：

1.Suffering from severe mental illness; 2.With severe cognitive impairment; 3.If the condition worsens and the study cannot be continued.
