研究目的：

1.对于肥胖合并胰岛素抵抗或2型糖尿的患者，比较术前使用司美格鲁肽与单纯手术在减重效果、胰岛素抵抗/2型糖尿病缓解、肥胖相关合并症改善等方面的差异； 2.探究术前体重减少量与术后及总体重减少量之间的关系； 3.探究术前使用司美格鲁肽减重在降低降低围手术期手术风险、减少住院时间等方面的效果。  

Objectives of Study：

1.Compare the differences between the preoperative use of semaglutide and surgery alone in terms of weight loss effects, insulin resistance/type 2 diabetes remission, and improvement of obesity-related comorbidities for patients with obesity combined with insulin resistance or type 2 diabetes; 2.Explore the relationship between preoperative weight loss and postoperative as well as overall weight loss; 3.Investigate the effects of preoperative weight loss using semaglutide on reducing perioperative surgical risks and shortening hospital stay.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄 16-65 岁，BMI≥27.5kg/m^2； 2.诊断为胰岛素抵抗（稳态模型评估胰岛素抵抗指数 HOMA-IR>2.69）或2型糖尿病（HOMA-IR=FPG×FINS/22.5）； 3.签署知情同意书且有我院完整随访资料

Inclusion criteria

1.Age 16-65 years, BMI >= 27.5 kg/m2; 2.Diagnosed with insulin resistance (Homeostasis Model Assessment of Insulin Resistance index HOMA-IR > 2.69) or type 2 diabetes (HOMA-IR = FPG × FINS / 22.5); 3.Signed informed consent and complete follow-up data from our hospital.

排除标准：

1.1型糖尿病、继发性糖尿病、妊娠期糖尿病； 2.既往行减重手术或入组前90天使用过其他减重药物或抽脂等医疗行为； 3.有急、慢性胰腺炎病史； 4.有甲状腺髓样癌（MTC）个人史或家族史； 5.精神类疾病控制不佳或拒绝参与随访评定

Exclusion criteria：

1. Type 1 diabetes, secondary diabetes, gestational diabetes; 2. History of weight loss surgery or use of other weight loss drugs or medical procedures such as liposuction within 90 days prior to enrollment; 3. History of acute or chronic pancreatitis; 4. Personal or family history of medullary thyroid carcinoma (MTC); 5. Poorly controlled mental disorders or refusal to participate in follow-up assessments.
