研究目的：

本研究旨在探讨一种基于学习通平台的线上线下混合式教育策略在出院后冠心病患者心脏康复中的应用及其效果评价，以期使患者出院后心脏康复自我管理保持较高水平，服药依从性、情绪状况及体适能均保持较好状态。 基于学习通平台的线上线下混合式教育将为临床医护人员对冠心病患者出院后心脏康复教育提供便捷、有效、可持续的策略，从而进一步推动心脏康复在社区的有效开展。  

Objectives of Study：

This study aims to explore the application and effect evaluation of an online and offline hybrid education strategy based on Xuetang platform in post-discharge coronary heart disease patients' cardiac rehabilitation, with the goal of maintaining a high level of self-management for patients' post-discharge cardiac rehabilitation, as well as good compliance with medication, emotional status, and physical fitness. The online and offline hybrid education strategy based on Xuetang platform will provide clinical medical staff with a convenient, effective, and sustainable strategy for post-discharge cardiac rehabilitation education of coronary heart disease patients, thereby further promoting the effective implementation of cardiac rehabilitation in the community.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.符合1979年国际心脏病学会和协会/世界卫生组织制定的冠心病诊断标准； 2.即将出院、在本市区居住； 3.认知、读写、理解能力正常； 4.拥有并使用智能手机； 5.精神状况良好，能进行口头和书面交流； 6.能够自如活动； 7.知情同意。

Inclusion criteria

1. Meet the diagnostic criteria for coronary heart disease established by the International Society of Cardiology and Association/World Health Organization in 1979; 2. Soon to be discharged from hospital, live in the city; 3. Normal cognitive, reading, writing and understanding abilities; 4. Owning and using a smartphone; 5. Good mental condition, able to communicate orally and in writing; 6. Be able to move freely; 7. Informed consent.

排除标准：

存在运动康复禁忌症，如不稳定性心绞痛、急性心肌梗死者、未控制的心律失常、失代偿性心力衰竭、复杂的室性心律失常、严重的肺动脉高压、腔内血栓、近期发生的血栓性静脉炎伴或不伴肺栓塞、严重的梗阻性心肌病、严重的或伴有症状的主动脉瓣狭窄、血压过高＞220mmHg、未控制的炎症或感染性疾病、任何阻碍运动训练的肌肉-骨骼肌疾病、合并严重肝肾功能损害、严重胃肠疾病、恶性肿瘤等的患者。

Exclusion criteria：

There are contraindications to exercise rehabilitation, Examples include unstable angina pectoris, acute myocardial infarction, uncontrolled arrhythmias, decompensated heart failure, complex ventricular arrhythmias, severe pulmonary hypertension, intracavitary thrombosis, recent thrombophlebitis with or without pulmonary embolism, severe obstructive cardiomyopathy, severe or symptomatic aortic stenosis, high blood pressure > 220mmHg, uncontrolled Patients with inflammatory or infectious diseases, any muscle-skeletal muscle diseases that hinder exercise training, complicated with severe liver and kidney function impairment, severe gastrointestinal diseases, malignant tumors, etc.
