纳入标准：

1）在医疗机构就诊的患者或患者陪护人员； 2）签署知情同意书时年龄≥18岁（以身份证上的出生日期为准）。

Inclusion criteria

1) Patients or patient companions visiting medical institutions; 2) Age >=18 years old when signing the informed consent form.

排除标准：

1）有精神疾病； 2）有其他限制行为能力的疾病。

Exclusion criteria：

1) Suffering from a mental disorder; 2) Other diseases that limit behavioral capacity.

本研究以参与的医疗机构为整群，将整群按照简单随机的方法分配到不同组。由一位与本项目无关的统计人员先将五家单位根据首字母的拼音顺序进行编号（1-5），然后使用Excel随机选取3个数字，将其代表的整群作为干预组，剩余2个数字代表的整群作为对照组。

Randomization Procedure (please state who generates the random number sequence and by what method)：

This study uses the participating medical institutions as clusters, and assigns these clusters to different groups by simple random sampling. First of all, an independent statistician five numbers (1-5) to these clusters according to the alphabetical order of their first letter in Pinyin, then uses Excel to randomly select three numbers. The clusters represented by these numbers are designated as the intervention group, and the remaining two numbers represent the control group.
