研究目的：

1.评价使用动物双歧杆菌乳亚种 BL-11 改善矮小儿童身高的有效性； 2.通过临床不良事件及实验室指标观察，评价动物双歧杆菌乳亚种 BL-11 改善矮小儿童身高的安全性。 3.评价动物双歧杆菌乳亚种 BL-11 干预过程中矮小儿童肠道菌群的变化。  

Objectives of Study：

1. To evaluate the effectiveness of using bifidobacterium animalis milk subspecies BL-11 to improve the height of short children; 2. By observing clinical adverse events and laboratory indicators, to evaluate the safety of animal bifidobacterium lactobacillus subspecies BL-11 in improving the height of short children; 3. To evaluate the changes of intestinal flora in dwarf children during the intervention of animal bifidobacterium lactobacillus subspecies BL-11.(YNMT)·

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.青春期前患者（Tanner I 期，男童 3 岁≤年龄＜9 岁，女童 3 岁≤年龄＜7.5 岁）；
2.体重：10kg≤体重≤40kg；
3. 监护人理解并签署知情同意书（需获得受试者的父母或法定代理人的知情同 意书,如受试者已经年满 8 岁也需签署知情同意书，不满 8 岁的受试者能够表达 同意时，其同意的意见应被明确记录）；
4. 近两个星期未服用抗生素等相关药物；近两周未服用益生菌；
5.有筛选前 6-12 个月内测量的历史身高值；
6.入组时 25-羟维生素 D 在 20ng/ml 以上；
1.特发性矮小症患儿需满足：1)出生时身长、体重处于同胎龄同性别婴儿的正常参考值第 10 百分位及以上；2)筛选时身高处于同年龄同性别正常儿童身高第 3 百分位以下； 3)排除系统性疾病、其他内分泌疾病、营养性疾病、染色体异常、骨骼发育不 良、心理情感障碍等其他原因导致身材矮小；4）两项不同药物 GH 激发试验证实 GH 峰值≥10.0ng/ml； 5）骨龄 (bone age，BA)－实足年龄（chronological age，CA）≤1 岁。
2.小于胎龄儿患者需满足：1）出生时体重和（或）身长低于同胎龄同性别正常参考值第 10 百分位的新生 儿；2）出生胎龄≥24 周；3）筛选时身高处于同年龄同性别正常儿童身高第 3 百分位以下。
3.Turner 综合征：1）染色体核型：45,X，45,X/46,XXqi，45,X/46,XXr，45,X/46,XX，46,XXqi， 46,XXpi，45,X/47,XXX，46,XXp-，45,X/46,XXp-，46,XXq-，45X/46,XXq-，45,X/46,XX/47,XXX 等；2）具有至少一项特殊躯体特征：包括但不限于后发际低、颜面部皮肤色素痣、 颈蹼、颈短、耳位低、小下颌、腭弓高、盾状胸、乳距宽、肘外翻、膝外翻、 第 4 及第 5 掌骨短、指甲发育不良、脊柱侧凸、上睑下垂、斜视、心血管系统 异常如主动脉狭窄、二叶式主动脉瓣、高血压、生殖系统异常如原发性腺功能 不全、肾脏畸形、甲状腺功能低下和中耳病变等；3）筛选时身高处于同年龄同性别正常儿童身高第 3 百分位以下。

Inclusion criteria

1. Pre-pubertal patients (Tanner stage I, boys 3 years old <= age < 9 years, girls 3 years old <= age < 7.5 years); 2. Weight: 10kg <= weight <= 40kg; 3. The guardian understands and signs the informed consent form (informed consent must be obtained from the subject's parents or legal representative, or if the subject is over 8 years of age, the informed consent form must also be signed; if the subject is under 8 years of age and is able to express consent, his/her consent should be clearly documented); 4. Have not taken antibiotics and other related medications in the last two weeks; have not taken probiotics in the last two weeks; 5. have historical height values measured within 6-12 months prior to screening; 6. 25-hydroxyvitamin D is above 20ng/ml at the time of enrollment; 1.Children with idiopathic dwarfism should meet the following requirements: 1) length and weight at birth are in the 10th percentile or above of the normal reference values for infants of the same gestational age and sex; 2) height at screening is below the 3rd percentile of the height of normal children of the same age and sex; 3) systemic diseases, other endocrine diseases, nutritional diseases, chromosomal abnormalities, skeletal developmental dysplasia, psycho-emotional disorders and other reasons are excluded. Short stature; 4) GH peak value >=10.0ng/ml confirmed by two different drugs GH stimulation test; 5) Bone age (BA)-chronological age (CA) <=1 year. 2.Patients with small-for-gestational-age infants must meet the following criteria: 1) birth weight and/or length below the 10th percentile of normal reference values for children of the same gestational age and sex; 2) birth gestational age >=24 weeks; and 3) height at screening below the 3rd percentile of height for children of the same age and sex. 3.Turner syndrome: 1) Karyotype: 45,X,45,X/46,XXqi,45,X/46,XXr,45,X/46,XX,46,XXqi,46,XXpi,45,X/47,XXX,46,XXp-,45,X/46,XXp-,46,XXq-, 45X/46,XXq-, 45X/46, 45Xq-, 45X/46,XXq-, etc; 2) Having at least one specific somatic feature: including low posterior hairline, facial skin pigmentation nevus, neck webbing, short neck, ear position, etc; 2) Having at least one specific somatic feature: including low posterior hairline, facial skin pigmentation nevus, neck webbing, short neck, short neck, short neck, ear position, etc. X/46,XX/47,XXX, etc.; 2) have at least one specific physical characteristic including, but not limited to, low posterior hairline, facial skin pigmented nevus, webbed neck, short neck, low-set ears, small jaw, high palatal arches, peltate, wide breast spacing, valgus, valgus, valgus, short 4th and 5th metacarpals, nail dysplasia, scoliosis, ptosis, squint, and cardiovascular system abnormalities such as aortic stenosis, bileaflet aortic valve, hypertension, reproductive system anomalies such as primary glandular insufficiency, renal malformations, hypothyroidism, and middle ear pathology, etc.; 3) Height below the 3rd percentile of normal height for children of the same age and sex at the time of screening.

排除标准：

1.小于胎龄儿：已确诊或疑似为 Bloom 综合征患儿；
2. Turner 综合征：体内含有 Y 染色体或有来源于 Y 染色体的片段；
3.骨骺已闭合者；
4.已确诊或高度怀疑生长激素分泌不足（growth hormone deficiency，GHD）患 者，及其他类型的生长异常：如 Noonan 综合征、Prader-Willi 综合征、 Russell-Silver 综合征等；
5.曾接受系统性（治疗时间超过 1 个月）促生长治疗者，包括但不限于 rhGH、 芳香化酶抑制剂、性激素等；
6.正在接受或需要长期给予可能对生长有影响的其他治疗，包括但不限于糖皮 质激素、哌甲酯；
7.肝肾功能异常值者（ALT＞1.5 ULN，Cr＞1 ULN）；
8.患有慢性感染性疾病：如慢性乙型肝炎、艾滋病或结核病；
9.严重的过敏体质或对生长激素或其辅料如甘露醇、赖氨酸、氯化钠等成分过 敏者；
10.既往患有恶性肿瘤病史或目前患有活动性恶性肿瘤者，包括颅内肿瘤；
11.糖调节异常者（包括空腹血糖异常和/或糖耐量异常）或糖尿病患者；
12. 精神病患者或有精神病家族史；
13.患有全身慢性疾病，如营养不良、免疫功能低下者、哮喘等；
14.先天性颅内高压者；
15.股骨头骨骺滑脱（SCFE）者；
16.脊柱侧凸超过 15°者；
17.筛选前 3 个月内（作为受试者）参加过任何药物临床试验且已接受药物干预者；
18.研究者认为具有不适宜参加本试验的其他因素者。

Exclusion criteria：

1. Small for gestational age: children with diagnosed or suspected Bloom syndrome；
2. Turner syndrome: the presence of Y chromosomes or segments derived from Y chromosomes;
3. Those whose epiphyses have closed;
4. Patients with diagnosed or highly suspected growth hormone deficiency (GHD) and other types of growth abnormalities: e.g., Noonan syndrome, Prader-Willi syndrome, Russell-Silver syndrome;
5. Those who have received systemic (more than 1 month) growth-promoting therapies, including but not limited to rhGH, aromatase inhibitors, and sex hormones;
6. Are receiving or require long-term administration of other treatments that may have an effect on growth, including but not limited to glucocorticoids, methylphenidate;
7. Those with abnormal values of liver and kidney function (ALT > 1.5 ULN, Cr > 1 ULN);
8. Suffer from chronic infectious diseases: such as chronic hepatitis B, AIDS or tuberculosis;
9. Severe allergies or hypersensitivity to growth hormone or its excipients such as mannitol, lysine, and sodium chloride;
10. Persons with a history of prior malignancy or current active malignancy, including intracranial tumors;
11. Individuals with abnormal glucose regulation (including abnormal fasting glucose and/or abnormal glucose tolerance) or diabetes mellitus;
12. Persons with mental illness or a family history of mental illness;
13. Suffer from chronic systemic diseases such as malnutrition, immunocompromised individuals, asthma, etc;
14. Those with congenital intracranial hypertension;
15. Those with slipped capital femoral epiphysis (SCFE);
16. Scoliosis greater than 15°;
17. those who have participated (as a subject) in a clinical trial of any drug within 3 months prior to screening and have received a pharmacologic intervention;
18. Those who, in the opinion of the investigator, have other factors that make participation in this trial inappropriate.
