研究目的:

本研究拟运用7T MRI技术观察糖尿病下肢病变的特征,并探究其对不同亚群糖尿病患者的影像特征差异识别能力。目标在于验证7T MRI能否提供强有力的影像学依据,以实现无创、全面且系统地诊断糖尿病下肢病变,帮助临床医师在糖尿病下肢病变的早期捕捉到病变进展的关键信息,通过早诊断、早治疗来改善糖尿病下肢病变的疗效及预后。  

Objectives of Study:

In this study, we plan to observe the characteristics of diabetic lower limb lesions using 7T MRI technology, and explore its ability to identify the differences in imaging features of different subgroups of diabetic patients. The objective is to verify whether 7T MRI can provide a strong imaging basis for the non-invasive, comprehensive and systematic diagnosis of diabetic lower limb lesions, help clinicians capture the key information of lesion progression in the early stage of diabetic lower limb lesions, and improve the efficacy and prognosis of diabetic lower limb lesions through early diagnosis and early treatment.

Description for medicine or protocol of treatment in detail:

 

纳入标准:

1. 糖尿病患者;
1)45-70岁,男女不限;
2)基于2023年美国糖尿病协会(ADA)标准诊断的糖尿病;
3)糖尿病下肢病变(符合以下任意一项): ①多伦多临床评分系统(TCSS)≥6分; ②神经电生理提示下肢存在1支以上神经(腓总神经、胫后神经和腓深神经)传导速度减慢(≥2个标准差),可伴有波幅下降; ③ 静息ABI≤ 0.90; ④ 超声多普勒或CTA检查下肢动脉有狭窄或闭塞病变;
4)自愿签署知情同意书。
2. 健康对照者;
1)年龄为45-70岁,男女不限;
2)基于2023年美国糖尿病协会(ADA)标准排除糖尿病诊断;
3)自愿签署知情同意书。

Inclusion criteria

1. Diabetic patients;
1) 45-70 years old, male or female;
2) Diabetes diagnosed based on 2023 American Diabetes Association (ADA) criteria;
3) Diabetic lower limb lesions (any of the following): ① Toronto Clinical Scoring System (TCSS) ≥ 6 points; ② nerve electrophysiology suggests that there are more than one nerve in the lower limb (common peroneal nerve, posterior tibial nerve and deep peroneal nerve) conduction velocity slowdown (≥ 2 standard deviations), which may be accompanied by a decrease in the amplitude of the wave; ③ resting ABI ≤ 0.90; ④ ultrasound Doppler or CTA examination of the arteries of the lower limbs have stenosis or occlusive lesions;
4) Voluntarily sign an informed consent form.
2. Healthy controls;
1) 45-70 years old, male or female;
2) Exclusion of a diagnosis of diabetes based on the 2023 American Diabetes Association (ADA) criteria;
3) Voluntarily sign an informed consent form.

排除标准:

1)其他原因所致神经病变,如颈腰椎病变(神经根压迫和椎管狭窄、颈腰椎退行性变等)、脑梗死、外伤等;
2)其他原因所致下肢血管病变,如多发性大动脉炎、动静脉瘘、遗传性毛细血管扩张症、毛细血管畸形等;
3)其他原因所致下肢肌肉病变,如炎性肌病、感染性肌病、肌萎缩侧索硬化症等;
4)行磁共振检查当天出现收缩压>180或舒张压>110mmHg或空腹血糖>14mmol/L;
5)各类原因引起的下肢水肿;
6)体内留有心脏支架、耳蜗假体、动脉瘤夹、金属植入物等其他MRI检查禁忌症;
7)AST/ALT >3倍于正常上限;
8)过去6个月曾患胆石症、急性胆囊炎、胰腺炎;
9)肾功能损害(eGFR <30mL/minute/1.73m2);
10)心力衰竭NYHA≥Ⅲ级;
11)12导联心电图有临床显著性心律失常、当前心律失常史、二度或三度心传导阻滞或左束支传导阻滞;
12)过去6个月内有心肌梗死;
13)过去12个月有TIA或中风史;
14)不适合参与临床试验的其它客观疾病进展,如肿瘤等疾病;
15)30天内参与其他临床试验;
16)近期(6月内)有生育计划的女性、妊娠或哺乳期妇女。

Exclusion criteria:

1) Neuropathy due to other causes, such as cervical and lumbar spine lesions (nerve root compression and spinal stenosis, cervical and lumbar degenerative changes, etc.), cerebral infarction, and trauma;
2) Lower extremity vascular lesions due to other causes, such as polyarteritis, arteriovenous fistula, hereditary capillary dilatation, capillary malformations, etc;
3) Lower limb muscle lesions due to other causes, such as inflammatory myopathies, infectious myopathies, and amyotrophic lateral sclerosis;
4) Systolic blood pressure > 180 or diastolic blood pressure > 110 mmHg or fasting blood glucose > 14 mmol/L on the day of MRI;
5) Lower extremity edema from all types of causes;
6) Other contraindications to MRI such as cardiac stents, cochlear implants, aneurysm clips, metal implants, etc., are retained in the body;
7) AST/ALT >3 times the upper limit of normal;
8) Have had cholelithiasis, acute cholecystitis, or pancreatitis in the past 6 months;
9) Renal impairment (eGFR <30mL/minute/1.73m2);
10) Heart failure NYHA ≥ class III;
11)12-lead ECG with clinically significant arrhythmia, current history of arrhythmia, second or third degree heart block, or left bundle branch block;
12) Myocardial infarction within the past 6 months;
13) History of TIA or stroke in the past 12 months;
14) Other objective disease progression, such as tumors and other diseases that are not suitable for participation in clinical trials;
15) Participation in other clinical trials within 30 days;
16) Women with recent (within 6 months) plans to have children, pregnant or breastfeeding women.

数据采集与管理:由具体负责本研究者填写《研究信息登记表》及影像数据的留取。 1. 病历、登记名簿、实验室检查的测定结果报告单、该临床观察专用记录资料包括医生临床病历、护理记录文书等均属于本研究中所要用到的资料; 2. 与临床观察有关的所有的资料、记录保存在研究者或临床试验医疗机构中,做到临床试验发起者或主管当局负责人可以直接阅览; 3. 临床观察研究者负责保存收集到的相关数据。除研究小组的成员、伦理委员会、医疗监管机构外,其他任何人若无正当理由,不能接触本研究收集到的数据。

Data collection and management: the person in charge of this study will fill in the “Research Information Registration Form” and retain the image data. 1. Medical records, registers, report cards of the results of laboratory tests, and records dedicated to the clinical observation, including the doctor's clinical records and nursing record documents, are all data to be used in this study; 2. all information and records related to the clinical observation are kept in the investigator's or the clinical trial's healthcare facility so that they are directly accessible to the sponsor of the clinical trial or the person in charge of the competent authority; 3. the investigator of the clinical observation is responsible for keeping the relevant data collected. No one other than the members of the research team, the ethics committee, and the healthcare regulatory agency will have access to the data collected in this study without a valid reason.
