研究目的：

本研究拟在运用CLCVP技术的腹腔镜肝切除术患者中运用更高水平的PHC，术中采用超声引导下ONSD间接反映允许性高碳酸血症对颅内压的影响，采用rSO2反映患者脑氧代谢情况，以ONSD、rSO2为指导，探寻最佳适宜的PHC，为临床上实施CLCVP技术腹腔镜肝切除患者提供脑保护方案。  

Objectives of Study：

This study intends to apply a higher level of PHC(Permissive hypercapnia) in patients undergoing laparoscopic hepatectomy using CLCVP(Controlled low central venous pressure) technology. During the operation, ONSD(optic nerve sheath diameter) under ultrasound guidance is used to indirectly reflect the influence of allowable hypercapnia on intracranial pressure, and rSO2 is used to reflect the cerebral oxygen metabolism of patients. With ONSD and rSO2 as guidance, the optimal and appropriate PHC is sought. To provide brain protection scheme for patients undergoing CLCVP laparoscopic hepatectomy.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①择期在控制性低中心静脉压下行腹腔镜肝切除术患者; ②体重在标准体重±15[标准体重（kg）=身高（cm）-100]; ③ASA分级为I-II 级，既往无异常手术麻醉史患者。

Inclusion criteria

① Patients undergoing laparoscopic hepatectomy under controlled low central venous pressure; ②Weight in the standard weight ±15[standard weight (kg) = height (cm) -100]; ③ Patients with ASA grade I-II and no previous history of abnormal surgical anesthesia.

排除标准：

①有脑血管疾病史、神经精神系统疾病、术前不能配合完成简易智力状态检查表 (MMSE)测评及 MMSE 评分<24分者。 ②有明显的心、肺、肝、肾功能障碍 ③高血压病控制不佳者及体位性低血压患者 ④有视神经及视神经鞘原发性疾病（如炎症、肿瘤）、眼部疾病（如青光眼）以及眼部创伤的患者。

Exclusion criteria：

① Have a history of cerebrovascular disease, neuropsychiatric disease, and can not cooperate with the completion of the MMSE assessment and MMSE score less than 24 points. ② There are obvious heart, lung, liver, kidney dysfunction ③ Patients with poorly controlled hypertension and postural hypotension (4) Patients with primary diseases of the optic nerve and optic nerve sheath (such as inflammation, tumors), eye diseases (such as glaucoma) and eye trauma.

测量方法：

术中采用高频线阵超声探头测量双眼ONSD，患者取仰卧位，双眼睑闭合，透明膜覆盖眼睑，涂上充足的硅胶使图像显示清晰，同时避免探头对眼球过分施压。扫描方向分为横断面和矢状面。视神经在超声图像中表现为球后管状低回声结构。根据国内外文献的推荐目前通常选择在眼球后3mm 的位置测量ONSD。
