研究目的：

研究在计划行肺切除术的高血压患者中，积极地运用肺复张联合个体化呼气末正压策略对血流动力学的影响。  

Objectives of Study：

To study the effect of lung recruitment maneuvers combined with individualized positive end-expiratory pressure (PEEP) on hemodynamics in hypertensive patients who planned to undergo pneumonectomy.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄≥18岁； 2.术前合并高血压1-3级； 3.ASA 1-3级； 4.预计单肺通气时间>60分钟

Inclusion criteria

1.age >=18 years; 2.preoperative hypertension grade 1-3; 3.ASA grade 1-3 4.expected duration of OLV >60 minutes

排除标准：

1.怀孕或哺乳； 2.急性呼吸窘迫综合征； 3.心力衰竭； 4.颅内高压； 5.过去15天内机械通气； 6.气胸或肺大泡； 7.慢性阻塞性肺疾病(COPD)需要吸氧或持续气道正压； 8.既往肺切除术； 9.同时参与另一项主要结局相似的介入研究；

Exclusion criteria：

1. Pregnancy or breastfeeding; 2.acute respiratory distress syndrome; 3.heart failure; 4.intracranial hypertension; 5.mechanical ventilation within the past 15 days; 6.pneumothorax or pulmonary bullae; 7.Chronic obstructive pulmonary disease (COPD) requiring oxygen or continuous positive airway pressure; 8.Previous pneumonectomy; 9.participating in another interventional study with similar primary outcome;

After written informed consent was obtained, eligible patients were randomly assigned, in a 1:1 ratio, to lung recruitment maneuvers with an individualized positive end-expiratory pressure strategy (individualized group) or to conventional protective ventilation (protective ventilation group). According to the classification of preoperative hypertension (group A: hypertension grade 1; Group B: hypertension stage 2) underwent central randomization
