研究目的：

建立北京协和医院自身免疫性小脑性共济失调患者的前瞻性、观察性队列； 探索 ACA 患者的病因类型构成、抗神经抗体谱，以及症状学、神经影像学等特点； 在 ACA 患者队列中探索和鉴定新的抗小脑抗体； 分析急性期免疫治疗、长程免疫治疗以及基于生物制剂的升级免疫治疗对ACA 患者急性期、长期预后及预防复发的意义。  

Objectives of Study：

To establish a prospective, observational cohort of patients with autoimmune cerebellar ataxia (ACA) at Peking Union Medical College Hospital; To explore the etiological types, anti-neural antibody spectra, and characteristics in symptomatology and neuroimaging among ACA patients; To investigate and identify novel anti-cerebellar antibodies within the ACA patient cohort; To analyze the efficacy of acute-phase immunotherapy, long-term immunotherapy, and monoclonal antibody therapy on the acute and long-term prognosis and prevention of relapse in ACA patients.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄≥ 3 岁；
2.符合北京协和医院调整的原发性自身免疫性小脑性共济失调诊断标准 ，或副肿瘤性小脑变性诊断标准（参考 Neurol Neuroimmunol Neuroinflamm 2021.8;21014），或抗 GAD 抗体相关小脑性共济失调诊断标准，以及以小脑性共济失调为主要表现的Miller Fisher 综合征、眼阵挛 肌阵挛综合征及自身免疫性脑炎患者；
3.患者或其监护人签署书面知情同意；

Inclusion criteria

1.Participants must be aged 3 years or older;
2.Participants must meet the diagnostic criteria for primary autoimmune cerebellar ataxia as revised by Peking Union Medical College Hospital, or the diagnostic criteria for paraneoplastic cerebellar degeneration (referencing Neurol Neuroimmunol Neuroinflamm 2021.8;21014), or the diagnostic criteria for GAD antibody-associated cerebellar ataxia. This also includes patients with cerebellar ataxia as the primary manifestation of Miller Fisher Syndrome, opsoclonus myoclonus ataxia syndrome, and autoimmune encephalitis;
3.Patients or their guardians must provide written informed consent;

排除标准：

1.确诊占位性病变、中枢神经系统脱髓鞘病、酒精中毒、遗传性疾病等其他病因相关的小脑性共济失调；
2.妊娠及哺乳期妇女；

Exclusion criteria：

1.Cerebellar ataxia associated with other etiologies such as space-occupying lesions, central nervous system demyelinating diseases, alcohol intoxication, genetic disorders;
2.Women who are pregnant or in the lactation period;
