研究目的：

在该多中心研究中，使用基于活细胞和固定细胞的MOG-IgG检测（CBA法），用于半定量检测高度疑似MOGAD（抗髓鞘少突胶质细胞糖蛋白自身免疫病）临床表现的研究参与者以及对照人群中人类血清样本中特异性抗体。每位研究参与者同时使用两种CBA检测方法来检测MOG-IgG，即活细胞基础和固定细胞基础，以获得两种方法的诊断性能参数和一致性评估（敏感性、特异性、阳性预测值、阴性预测值），并结合全面的临床诊断来比较它们的临床诊断价值。旨在探索中国MOGAD诊断的个体检测截断值。  

Objectives of Study：

In this multicenter study, a cell-based assay (CBA) based on cell transfection was proposed to semi-quantitatively detect specific antibodies in human serum samples of study participants with highly suspected clinical manifestations of MOGAD and the control population. Each study participant used two CBA detection methods to detect MOG-IgG at the same time, namely live cell based and fixed cell based, to obtain diagnostic performance parameters and consistency evaluation (sensitivity, specificity, positive predictive value, negative predictive value) of the two methods, and combined with comprehensive clinical diagnosis to compare their clinical diagnostic value. To explore the individual assay cutoffs for the diagnosis of MOGAD in China.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄≥18周岁，男女不限；
2.MOGAD高度疑似；
3.其他炎症性CNS疾病对照组包括：依据各疾病诊断标准及共识，明确诊断的多发性硬化症, 自身免疫性脑炎（除外NMDAR脑炎），格林－巴利综合征(GBS)， 慢性炎性脱髓鞘性多发性神经根神经病(CIDP)， 视网膜耳蜗脑血管病变(SUSAC)，POEMS综合征(POEMS)，意义未明的单克隆丙种球蛋白血症(mGUS)，结节病(Sarcoidosis)等；
4.非炎症性中枢神经系统疾病对照组包括：偏头痛(Migraine)，脑小血管病(CSVD)，良性颅高压，胶质瘤(Glioma)明确诊断且无其它自身免疫性疾病的患者；
5.健康对照包括通过体检等方式明确无自身免疫性疾病的健康人群；
6.临床资料完整；
7.已获得患者或其监护人知情同意；

Inclusion criteria

1.Age ≥18 years old, male and female.
2.MOGAD is highly suspected.
3.Other inflammatory CNS disease control groups include: According to the diagnostic criteria and consensus of various diseases, the diagnosis of multiple sclerosis, Autoimmune encephalitis (except NMDAR encephalitis), Guillain-Barre syndrome (GBS), Chronic Inflammatory Demyelinating polyradiculopathy (CIDP), Retinal Cerebrovascular disease (SUSAC), POEMS syndrome (POEMS), and neuropathy were confirmed. The monoclonal gammopathy of unknown significance (MGUS), Sarcoidosis and so on.
4.The control group of non-inflammatory central nervous system diseases included: Migraine, CSVD, benign cranial hypertension, Glioma with definite diagnosis and no other autoimmune diseases.
5.Healthy controls include healthy people who have no autoimmune diseases through physical examination and other means.
6.Complete clinical data.
7.Informed consent of the patient or his guardian has been obtained.

排除标准：

1.依据《视神经脊髓炎谱系疾病诊断与治疗指南（2021版）》，明确诊断为AQP4阳性的视神经炎谱系疾病(NMOSD)患者。
2.依据《中国自身免疫性脑炎诊治专家共识（2022版）》，明确诊断为 NMDAR脑炎患者。
3.血清和（或）脑脊液抗胶质纤维酸性蛋白抗体(GFAP-IgG)阳性患者。
4.临床资料缺失（所需临床资料详见3.5.2临床信息收集）。
5.血液样本不合格。
6.未获得患者知情同意。
7.研究过程中误诊误入本研究。

Exclusion criteria：

1.According to the Guidelines for Diagnosis and Treatment of Optic Neuromyelitis Spectrum Diseases (2021), patients with optic Neuromyelitis Spectrum diseases (NMOSD) with positive AQP4 were clearly diagnosed.
2.According to the Expert Consensus on the Diagnosis and Treatment of Autoimmune encephalitis in China (2022), the diagnosis was confirmed as NMDAR encephalitis.
3.Patients with positive anti-glial fibrillary acidic protein antibody (GFAP-IgG) in serum and/or cerebrospinal fluid.
4.Lack of clinical data.
5.Unqualified blood samples.
6.The patient's informed consent was not obtained.
7.Misdiagnosis in the research process went wrong in this study.
