探讨失眠认知行为疗法联合耳穴贴压及乐眠操对医养结合机构中老年睡眠障碍患者的干预效果。  

Objectives of Study：

To investigate the intervention effect of cognitive behavioral therapy for insomnia combined with auricular acupressure and Le Sleep Exercise on middle-aged and elderly patients with sleep disorders in health care institutions.

纳入标准：

纳入标准：匹兹堡睡眠质量指数得分（Pittsburg Sleep Quality Index，PSQI）≥7 分；年龄≥65 岁；有正常沟通能力；自愿参加本研究。

Inclusion criteria

Inclusion criteria: a Pittsburg Sleep Quality Index (PSQI) score of ≥7; age ≥65 years; normal communication skills; and voluntary participation in this study.

排除标准：

排除标准：有严重的躯体障碍；严重器质性精神障碍或正在接受精神性治疗者；正参加其他研究者；中途不愿意继续接受干预和调查的患者。

Exclusion criteria：

Exclusion criteria: those with severe somatic disorders; those with severe organic mental disorders or undergoing psychosexual treatment; those who were participating in other studies; and patients who were unwilling to continue with the intervention and survey midway through the study.

以小组为单位（每组10名患者）进行失眠认知行为疗法，护士带领患者观看睡眠科普视频、讲解健康睡眠知识、进行放松训练；每周一次（每周三下午3点开始），每次45-60min；干预12周

在CBT-I的基础上对患者进行耳穴贴压治疗干预。护士评估患者局部皮肤是否完好以及身体有无不适后可进行耳穴贴压操作。穴位选择：神门、内分泌、交感神经、皮质下以及心作为主要耳部穴位。医护人员进行耳穴贴位置选择。耳穴贴每 5天-7天进行1次更换，两侧耳廓更替施治，干预需持续 12 周。

On the basis of CBT-I, the patients were treated with auricular point sticking. Nurses can perform auricular point sticking after assessing whether the patient's local skin is intact and whether the body is uncomfortable. Acupoint selection: Shenmen, endocrine, sympathetic nerve, subcortical and heart were selected as the main auricular acupoints. The location of auricular plaster was selected by medical staff. Auricular plaster was replaced every 5 to 7 days, and the auricles on both sides were replaced. The intervention lasted for 12 weeks.

在CBT-I的基础上进行乐眠操的指导学习。乐眠操方法如下：预备：患者充分热身，保证身体处于相对放松状态；开始练习：主要穴位——气海、命门、大椎、百会、神庭、檀中；各穴位 200~300 下/次。做乐眠操的具体时间为每天早上九点，患者以养老楼层为单位集中进行，由1 名接受过培训的护士带领，另 1 名护士监督、记录患者完成情况。

The method of Lemian exercise is as follows: Preparation: the patient is fully warmed up to ensure that the body is in a relatively relaxed state; Start to practice: the main acupoints: Qihai, Mingmen, Dazhui, Baihui, Shenting, Danzhong; Each acupoint was applied 200-300 times/time. The specific time of Le Mian exercise was 9 o 'clock every morning. The patients were concentrated in the pension floor as a unit, led by a trained nurse, and another nurse supervised and recorded the completion of the patient.

研究者先通过随机数字法抽取该院的6个养老楼层，为便于分组与记录，将6个养老楼层按1-6进行编号；为避免三组患者相互干扰、污染，研究者基于养老楼层进行整群随机分组，将6个养老楼层按照随机抽签法分为三个干预组，1、6养老楼层作为A组，2、3养老楼层作为B组，4、5养老楼层作为C组。

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：
