研究目的：

评价基于EIT技术的ICU机械通气患者的个体化气道廓清方案与常规气道廓清技术应用效果差异。  

Objectives of Study：

To evaluate the differences in the application of individualized airway clearance protocols in ICU mechanically ventilated patients based on EIT technology and conventional airway clearance techniques.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①年龄≥18岁；②血流动力学稳定；③进行机械通气存在人工气道和气道廓清障碍患者：听诊痰鸣音、无效咳嗽(咳嗽评分<3分)、分泌物量明显增多>30mL/d、血氧饱和度降低、发绀、呼吸困难等痰液潴留指征；④患者家属知情同意。

Inclusion criteria

(1) Age≥ 18 years old; (2) hemodynamically stable; (3) Patients with artificial airway and airway clearance disorders after mechanical ventilation: sputum retention indications such as sputum sounds, ineffective cough (cough score < 3 points), significantly increased secretion volume >30mL/day, decreased oxygen saturation, cyanosis, dyspnea and other indications of sputum retention; (4) Informed consent of the patient's family.

排除标准：

①有气道廓清技术治疗禁忌证，如：伴有心肺功能严重不全；胸部皮肤破损、胸部皮下感染患者、胸部骨折；妊娠患者；不稳定性静脉血栓或肺动脉栓塞、未经引流的气胸；②无法使用EIT技术，如：胸部皮肤破损、胸部皮下感染患者、胸部骨折；严重行为障碍，躁动不配合患者；植入心脏起搏器；③BMI<18，BMI>50。

Exclusion criteria：

(1) There are contraindications to the treatment of airway clearance techniques, such as: accompanied by severe cardiopulmonary insufficiency; Chest skin breakage, patients with chest subcutaneous infection, chest fracture; Pregnant patients; unstable venous thrombosis or pulmonary embolism, undrained pneumothorax; (2) Inability to use EIT technology, such as: chest skin breakage, chest subcutaneous infection patients, chest fractures; Severe behavioral disorder, agitation and uncooperative patients; implantation of a pacemaker; ③BMI<18,BMI>50.

根据肺部听诊、X线、CT检查结果，给予对照组ICU常规的气道廓清方案，包括雾化、体位引流、气道内吸痰、震动排痰（气道内震荡或体外胸壁震荡）等，治疗过程中患者出现不适、病情变化、氧合下降等，立即终止并查明原因针对性处理。

在对照组的基础上，呼吸治疗师每日使用EIT进行评估。根据区域性肺通气结果予对应的气道廓清方案，之后每日进行评估和动态调整气道廓清方案，直至患者撤离呼吸机，拔除人工气道。

On a control basis, respiratory therapists were assessed daily using an EIT. The airway clearance protocol is given according to the results of regional lung ventilation, and then the airway clearance protocol is evaluated and dynamically adjusted daily until the patient is evacuated from the ventilator and the artificial airway is removed.
