研究目的：

探讨超高压球囊直径阶梯式扩张治疗桡动脉-头静脉自体动静脉内瘘( RCAVF )再发狭窄的临床效果。  

Objectives of Study：

To investigate the clinical effect of ultrahigh-pressure balloon diameter stepped dilatation in the treatment of recurrent stenosis of radio-cephalic autologous arteriovenous fistula .

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

( 1 )年龄18 ~ 80岁。( 2 )均为成熟的RCAVF，狭窄至少3次。( 3 )选取所有I型狭窄(狭窄发生在动静脉吻合口及吻合口2cm内)的患者。（4）符合2019中国血液透析用血管通路专家共识（第2版）AVF狭窄干预指征 ）局部狭窄率超过附近正常血管管径的50%并伴以下情况：内瘘自然血流量＜500 mL/min；不能满足透析时所需血流量；透析静脉压升高；穿刺困难；透析充分性下降；以及内瘘出现异常体征（吻合口震颤减弱或消失、远端静脉张力高、搏动等）。

Inclusion criteria

(1) Age 18-80 years old. (2) All of them were mature RCAVF and stenosis at least 3 times.(3) All patients with type I stenosis (stenosis occurring at arteriovenous anastomosis and within 2cm of anastomosis) were selected. (4) It is consistent with the AVF stenosis intervention indication in the Expert Consensus on Vascular Access for hemodialysis in China (2nd edition) (The local stenosis rate exceeding 50% of nearby normal vascular diameter accompanied by the following conditions: internal fistula natural blood flow < 500mL/min; Can not meet the blood flow required for dialysis; Increased dialysis venous pressure; Puncture difficulty; Decreased dialysis adequacy; And abnormal signs of internal fistula (decreased or disappeared anastomotic tremor, high distal venous tension, pulsation, etc.).

排除标准：

（1）心功能Ⅳ级，左室射血分数小于30%，合并严重心脑等重要脏器功能障碍患者。（2）恶性肿瘤。（3）严重肝病。（4）内瘘感染。（5）手术血管曾有血管病变以及血管存在变异的患者。（6）应用药物球囊、切割球囊或其他特殊类型球囊。（7）存在精神异常、认知障碍而无法配合的患者。

Exclusion criteria：

(1) Patients with heart function grade IV, left ventricular ejection fraction less than 30%, and serious dysfunction of heart and brain and other important organs. (2) Malignant tumors. (3) Severe liver disease. (4) Internal fistula infection. (5) Patients with vascular disease and vascular variation before surgery. (6) The application of drug balloons, cutting balloons or other special types of balloons. (7) Patients with mental abnormalities and cognitive disorders who are unable to cooperate.
