研究目的:

通过初发GD患者的甲状腺组织、外周血的单细胞转录组学、免疫组学研究,剖析初发GD患者的甲状腺和外周免疫微环境特点,进一步揭示GD致病的分子免疫机制,为针对GD的免疫病因治疗提供科学的理论指导和研究基础。  

Objectives of Study:

By studying the single-cell transcriptomics and immunohistochemistry of thyroid tissue and peripheral blood in newly diagnosed GD patients, the characteristics of thyroid and peripheral immune microenvironment in newly diagnosed GD patients are analyzed, and the molecular immune mechanism of GD pathogenesis is further revealed, providing scientific theoretical guidance and research basis for the treatment of GD immunological etiology.

Description for medicine or protocol of treatment in detail:

 

纳入标准:

(1)病例组:确诊GD,病程≤3月,甲功控制正常/接近正常。同时满足以下条件之一: ①伴有单个高风险结节,结节直径<1.0cm; ②甲状腺Ⅱ-Ⅲ°肿大,有压迫症状或严重影响外观; ③中、重度甲亢,疗效不佳; ④ATD服药出现严重不良反应或不能坚持服药; ⑤胸骨后甲状腺肿; ⑥重度Graves眼病,需要快速降低TRAb。 (2)对照组: ①年龄、性别与病例组匹配; ②甲状腺微小乳头状癌,单病灶; ③无桥本甲状腺炎及其他自身免疫性疾病。

Inclusion criteria

(1) Case group: Diagnosed with GD, disease duration ≤ 3 months, normal/near normal control of thyroid function. Simultaneously meet one of the following conditions: ① Accompanied by a single high-risk nodule with a diameter of less than 1.0cm; ② Thyroid II-III degree enlargement with compression symptoms or severe impact on appearance; ③ Moderate to severe hyperthyroidism with poor therapeutic effect; ④ Serious adverse reactions or inability to persist in taking ATD medication; ⑤ Posterior sternal goiter; ⑥Patients with severe Graves orbital disease require rapid reduction of TRAb. (2) Control group: ① Age and sex can be matched with case group; ② Thyroid papillary carcinoma, single lesion; ③ No Hashimoto's thyroiditis or other autoimmune diseases.

排除标准:

(1)合并严重心、肝、肾疾病不能耐受手术者; (2)妊娠女性; (3)未签署知情同意书及不能6月后复诊者。

Exclusion criteria:

(1) Patients with severe heart, liver, and kidney diseases who cannot tolerate surgery; (2) Pregnant women; (3) Not signing the informed consent form and not being able to follow up after 6 months.
