研究目的:

脑卒中是常见的脑血管疾病,具有高病死率和高致残率的特点。吞咽障碍是脑卒中后常见并发症之一,临床表现为进食时咀嚼无力,吞咽时呛咳,吞咽后食物残留于口腔内或从口角漏出,或吞咽完毕后反流。吞咽功能障碍会导致肺炎引起脱水、营养不良,甚至因窒息而危及生命。在我国,急性脑卒中后吞咽障碍发生率为64%~78%,在美国,42%-67%的脑卒中后患者3d内会出现吞咽问题。吞咽障碍会威胁患者气道安全,导致误吸和吸入性肺炎,也会造成患者进食量减少、脱水、电解质紊乱及营养不良。中华人民共和国卫生行业WS/T558-2017《脑卒中患者膳食指导》指出,改进食物性状是脑卒中吞咽障碍的主要治疗方法之一。 IDDSI国际膳食标准,是由国际吞咽障碍食物标准行动委员会制定的吞咽障碍食物国际标准,准确描述了适用于不同文化和不同年龄层吞咽障碍病人的食物质地和增稠饮品。 基于IDDSI饮食指导的膳食管理,是我科创建的基于IDDSI饮食指导的脑卒中吞咽障碍患者膳食管理模式,包括脑卒中吞咽障碍患者吞咽功能评估,饮食途径选择,饮食计划制定,餐具选择,健康宣教等方面进行膳食管理。 本研究探讨基于IDDSI饮食指导的膳食管理在脑卒中吞咽障碍病人中的应用,期望能改善患者吞咽功能,保障病人安全进食,促进病人的康复,减少相关并发症,改善病人的预后,提高脑卒中吞咽障碍病人生存质量与满意度。希望为我国脑卒中后吞咽障碍病人提供更多的饮食选择,为膳食管理提供数据支持。  

Objectives of Study:

Stroke is a common cerebrovascular disease with high mortality and disability. Dysphagia is one of the common complications after stroke. The clinical manifestations are weak mastication when eating, choking and coughing when swallowing, food remaining in the mouth or leaking from the angle of mouth after swallowing, or regurgitation after swallowing. Dysphagia can lead to dehydration, malnutrition and even life-threatening asphyxiation due to pneumonia. In our country, the incidence of dysphagia after acute stroke ranges from 64% to 78% , and in the United States, 42% to 67% of patients with post-stroke dysphagia develop swallowing problems within 3 days. Dysphagia can compromise the airway and lead to aspiration and aspiration pneumonia. It can also lead to reduced food intake, dehydration, electrolyte disturbances and malnutrition. According to WS/T558 -2017 dietary guidelines for stroke patients, improving food traits is one of the main treatments for dysphagia after stroke. IDDSI is the International standard for food with Dysphagia developed by the International Dysphagia Diet Standardisation Initiative (IDDSI) , accurately describes the texture of foods and thickened beverages suitable for patients with dysphagia of different cultures and ages. Diet management based on IDDSI diet guidance is a model of diet management based on IDDSI diet guidance for stroke patients with deglutition disorder, which includes deglutition function assessment, dietary pathway sele.

纳入标准:

1.符合中国各类主要脑血管病诊断要点,并经头颅CT或 MRI确诊的脑卒中患者;
2.符合脑卒中后吞咽障碍诊断标准;
3.患者生命体征平稳,意识清楚,病情处于稳定期可简单沟通交流,能配合治疗及完成各项评分;
4.病人及家属签署知情同意书;

Inclusion criteria

1.Stroke patients who meet the diagnostic criteria for various major cerebrovascular diseases in China and have been confirmed by head CT or MRI;
2.Meets the diagnostic criteria for post-stroke dysphagia;
3.The patient's vital signs are stable, their consciousness is clear, and their condition is in a stable period. They can communicate and cooperate with treatment to complete various scoring tasks;
4.Patients and family members signed informed consent forms;

排除标准:

1.意识不清者,有心、肝 、肾、造血系统严重并发症者及精神症状患者;
2.有重要器质性疾病、吞咽障碍、明显咽喉、口腔疾病既往史患者;
3.依从性差不能配合治疗及完成评分;

Exclusion criteria:

1.Individuals with unclear consciousness, severe complications of the heart, liver, kidneys, hematopoietic system, and psychiatric symptoms;
2.Patients with significant organic diseases, swallowing disorders, obvious history of throat and oral diseases;
3.Poor compliance and inability to cooperate with treatment and complete scoring;
