研究目的：

构建“医疗-口腔”协同模式（MDI）背景下，开发适合在基层医疗机构儿保常规工作开展儿童龋风险管理（CRAM）内容及流程；明确儿保医生执行CRAM的阻碍因素；评估儿保医生执行CRAM的保真度和持续性；评估家长执行儿童口腔家庭护理行为的改变。  

Objectives of Study：

Under the background of the "Medical-Dentlal" collaborative model (MDI), this study will develop the content and process of child caries risk management (CRAM) which is suitable for the routine work of primary child physicians (PCP) in primary care institutions. Barriers, facilitators, fidelity, and sustainability of implementing CRAM by PCP will be identified. Additionally, family dental care behavior change will be evaluated.

药物成份或治疗方案详述：

本研究为阶梯楔形设计(Stepped Wedge Design, SWD)的整群随机对照试验，将在泸州市3个区县，每个区县选择3个承担儿保基本公共卫生服务的妇幼保健院或社区卫生服务中心，一共9个基层医疗机构。随机将其分为3个组，分3阶段对每个组进行干预，所有的分组都将依次进入干预组。每阶段为间隔约6个月，加上基线观测，总观察期为4期。 

Description for medicine or protocol of treatment in detail：

This study is a cluster randomized controlled trial with Stepped Wedge Design (SWD). In 3 districts and counties of Luzhou City, 3 maternal and child health hospitals or community health service centers with basic public health services for children will be selected in each county, and a total of 9 primary medical institutions will be established. They were randomly divided into 3 groups, and each group was subjected to intervention in 3 stages, and all groups would enter the intervention group in turn. The interval of each stage was about 6 months, and the total observation period was 4 periods with baseline observation. 

纳入标准：

1. 儿保医生：临床一线从事儿童保健的医务人员； 2. 12-18个月性别不限幼儿及其家长。

Inclusion criteria

1. Primary care physicians: medical personnel engaged in child health care at the clinical line; 2. Children aged 12-18 months are of either gender and their parents.

排除标准：

1. 排除长期未在岗的儿保医生； 2. 排除有基础疾病的儿童；不愿配合的儿童。

Exclusion criteria：

1. Exclude primary care physicians who have been absent for a long time; 2. Exclude children with underlying medical conditions; Children who won't cooperate.
