研究目的：

探讨基于HAPA理论的运动干预对乳腺癌患者早期肿瘤药物心脏毒性相关监测指标的影响，旨在预防乳腺癌患者肿瘤药物心脏毒性，并提高其体力活动水平、运动自我效能、锻炼社会支持及运动依从性。  

Objectives of Study：

This study aims to assess the effects of a 4-month HAPA model based exercise protocol on cardiotoxicity related monitoring indicators, physical activity level, exercise self-efficacy, exercise social support and exercise compliance through a quasi-randomised controlled trial (Q-RCT).

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）病理诊断为乳腺癌I-III期；（2）年龄18-65岁；（3）首次接受抗肿瘤药物治疗，治疗方案包含蒽环类药物；（4）经评估健康行为阶段处于前意向阶段；（5）具有良好的沟通理解能力，知情同意，自愿参与研究。

Inclusion criteria

(1) pathological diagnosis of stage I-III BC; (2) age 18-65 years; (3) receiving chemotherapy for the first time with a Anthracycline regimen; (4) being in preintention stage assessed by health behavior stage assessment questionnaire; (5) having good communication and understanding skills, voluntary provision of informed consent to participate in this study.

排除标准：

（1）既往确诊为心脑血管疾病，如冠心病、心力衰竭、心律失常、脑卒中等；（2）存在运动禁忌症，如哮喘、严重贫血等；（3）肿瘤转移或复发；（4）目前或既往有精神、心理方面的疾病。

Exclusion criteria：

(1) previously diagnosed with cardiovascular and cerebrovascular disease, such as coronary heart disease, heart failure, arrhythmia, stroke, etc, (2) exercise contraindications, such as asthma, severe anemia, etc, (3) tumor metastasis or recurrence; (4) present or previous mental or psychological illness.

(1）常规护理：①健康宣教：由责任护士为患者介绍所用药物及药物使用过程中的注意事项，指导患者出现药物不良反应的正确处理措施，密切观察病情变化；②饮食护理：指导患者正确饮食，禁忌辛辣刺激食物，多吃高维生素、高蛋白、易消化食物，多饮水；③心理护理：观察患者的情绪变化，做好心理护理，消除患者的情感顾虑，并指导家属给予情感支持与帮助； (2)运动宣教：患者首次入院接受抗肿瘤药物治疗期间，研究者向患者发放《“乳动心康”运动手册》，告知患者出院后遵循手册中的“乳动心康运动方案”进行运动。每月对患者进行随访了解其运动状态，如果患者没有坚持运动，研究者询问患者原因，帮助并鼓励他们坚持运动。

Intervention：

Usual care and exercise guidance will be deliverd to the control group. Usual care will contain: (1) health education: introduce the antineoplastic drugs and the precautions in the process of infusion to the patients and guide them correct measures of adverse drug reactions, and closely observe the changes of the condition;(2) diet instruction: avoid spicy and stimulating food, eat more high-vitamin, high-protein, digestible food, drink more water; (3) psychological care: pay attetion to patients' emotional changes to eliminate their emotional worries and guide the family members to provide emotional support and help. Exercise guidance: when patients are admitted to the hospital for their first chemotherapy treatment, the researcher will distribute the exercise instruction manuals to patients and inform them to follow the exercise training protocol described in the manual. The patients will be followed up monthly to learn their exercise status, and if patients have not persisted in following the recommendations, the researcher will ask patients why in order to help solve their problems and to encourage them to persist in exercise training.

实施常规护理与基于HAPA理论的运动干预方案： 前意向阶段（第1-2周） 第一周：①向患者发放《“乳动心康”运动手册》，依据手册向患者介绍肿瘤药物的心脏毒性、心脏毒性的不良后果，增强患者对心脏毒性的风险感知；②依据手册向患者讲解运动锻炼对于心血管功能的好处，强调规律的运动在预防心脏毒性中的积极作用，让患者期待运动产生的积极结果；③添加患者微信，每周推送有关肿瘤药物心脏毒性、运动相关信息； 第二周：①分享积极运动、预后良好的病友案例，树立患者信心，激发自我效能；②线上推送肿瘤药物的心脏毒性不良后果、运动对心脏毒性的益处相关文章。 意向阶段（第3-4周） 第三周：依据手册为患者介绍“乳动心康”运动方案及注意事项； 第四周：根据患者的运动偏好，依据运动方案与患者共同制定运动计划；线上推送有氧运动及抗阻训练的相关信息。 行动阶段（第5-16周） ①每周在微信提醒督促患者锻炼并要求患者提供运动记录视频，运动中断时及时联系患者询问原因，保证运动尽快恢复； ②获得家属的支持与帮助，指导家属在日常生活中鼓励患者运动，并发挥在患者运动时的监督及陪伴作用，增强患者的自我效能感； ③每周推送运动相关信息，鼓励患者积极提问，研究者及时回答患者的相关问题，对于积极运动的患者予以表扬并邀请其分享运动经验； ④每月对患者进行电话或面对面的随访，询问患者运动过程中遇到的问题，针对他们的问题提供个性化指导策略，适时调整运动计划。

Intervention：

The intervention group will receive usual care and HAPA model based exercise protocol. Preintention stage Week 1: (1) Distribute exercise manuals to patients, according to which cardiotoxicity and its adverse consequences will be introduced to patients to enhance their risk perception of cardiotoxicity; (2) Explain patients the benefits of exercise for cardiovascular function according to the manual, and emphasize the positive role of regular exercise in preventing cardiotoxicity, which would let patients look forward to the positive results of exercise; (3) Add patients' Wechat account, information about cardiotoxicity and exercise will be delivered every week. Week 2: (1) Share cases of good prognosis due to active exercise to build patients' confidence and stimulate their self-efficacy; (2) The adverse consequences of cardiotoxicity and the benefits of exercise on cardiotoxicity will be pushed in the wechat group. Intention stage Week 3: Introduce the exercise program and precautions to the patient according to the manual. Week 4: Make exercise plan with the patient depending on their exercise preference; information about aerobics and resistance training will be also available online. Action stage Week5-16: (1) Remind and urge patients to exercise on wechat every week and ask them to provide video recording of exercise, contact them in time to ask the reasons when the exercise is interrupted to ensure their exercise can resume as soon as possible; (2) Get support from family members and instruct them to encourage patients to exercise in daily life, and play their role of supervision and companionship during exercise to enhance patients' self-efficacy; (3) Release exercise-related information every week, encourage patients to actively ask questions on the Wechat, the researcher will timely answer the relevant questions, patients who actively exercise will be praised and invited to share their exercise experience; (4) The researchers will follow up the patients by phone or face to face every month to ask their problems encountered in the exercise process and provide personalized guidance strategies, the exercise plan will be adjusted in time if necessary.
