研究目的：

了解针刺干预脑卒中后上肢痉挛性瘫痪的生物力学效应机制。  

Objectives of Study：

To understand the biomechanical mechanism of acupuncture intervention in upper limb spastic paralysis after stroke.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 符合脑梗死或脑出血的西医诊断标准；
2. 因脑卒中后引起的中重度运动功能障碍（上肢简化中文版Fugl-meyer量表评分≥16，且≤50分）；同时上肢肩关节、肘关节、腕关节、掌指关节任意一处存在肌张力增高， 即改良Ashworth评级(Modified Ashworth Scale, MAS)评级在Ⅰ-Ⅲ级；
3. 20岁≤年龄≤80岁，性别不限；
4. 脑卒中发病后病程超过14天；
5. 既往无脑卒中发病史；
6. 病情基本稳定，意识清醒，生命体征平稳；
7. 能独立维持坐姿20分钟以上者；
8. 理解、同意参加本研究并签署知情同意书。

Inclusion criteria

1. Conform to the western diagnostic criteria for cerebral infarction or cerebral hemorrhage;
2. Moderate and severe motor dysfunction caused by stroke (the score of the simplified Chinese version of Fugl meyer scale for upper limbs: 16-50); at the same time, the muscular tension of the shoulder joint, elbow joint, wrist joint and metacarpophalangeal joint of the upper limb is increased, that is, the modified Ashworth Scale (MAS) is rated at Grade I - III;
3. Aged 20-80 years, regardless of gender;
4. The course of stroke exceeds 14 days;
5. No history of stroke in the past;
6. The patient's condition is basically stable, consciousness is clear, and vital signs are stable;
7. Can independently maintain sitting posture for more than 20 minutes;
8. Understand and agree to participate in this study and sign the informed consent form.

排除标准：

1. 非脑血管意外引起的上肢运动功能障碍及肌张力增高者；
2. 入组前2周使用过骨骼肌松弛剂者或镇静类药物者；
3. 患有严重凝血功能障碍疾病者；
4. 入组前1月内接受过针灸改善上肢运动功能者；
5. 视力、听觉障碍及认知功能严重障碍导致不能配合康复治疗及评估者；
6. 健侧存在运动功能障碍或入组前1月内健侧上肢或肩背部有过外伤或疼痛或感觉障碍病史。

Exclusion criteria：

1. Upper limb motor dysfunction and muscular tension increase not caused by cerebrovascular accident;
2. Those who used skeletal muscle relaxants or sedatives 2 weeks before enrollment;
3. Patients with severe coagulation dysfunction;
4. Those who have received acupuncture and moxibustion to improve upper limb motor function within one month before enrollment;
5. Those who cannot cooperate with rehabilitation treatment and evaluation due to visual, auditory and cognitive impairment;
6. The contralateral side has motor dysfunction or has a history of trauma, pain or sensory disturbance in the contralateral upper limb or shoulder and back within 1 month before enrollment.
