研究目的：

观察EVA、PETG-TPU材质以及不同厚度EVA全牙列弹性??垫在牙周基础治疗阶段对Ⅲ期/Ⅳ期牙周炎患者松动后牙的治疗效果，同时评价两种??垫辅助牙周非手术治疗过程中对龈沟液内MMP-8、OPG及RANKL水平的影响，为全牙列弹性??垫应用于Ⅲ期/Ⅳ期牙周炎患者松动后牙提供理论依据。  

Objectives of Study：

This study aims to investigate the therapeutic effects of EVA and PETG-TPU materials, as well as EVA elastic occlusal splints of varying thicknesses, on the stabilization of loose posterior teeth in patients with stage III or IV periodontitis during the initial phase of periodontal treatment. Additionally, the study seeks to evaluate the impact of these two types of occlusal splints on the levels of MMP-8, OPG, and RANKL in gingival crevicular fluid during non-surgical periodontal therapy. The findings are intended to provide a theoretical foundation for the application of full dentition elastic occlusal splints in managing loose posterior teeth in patients with advanced periodontitis.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）年龄18-70岁； （2）Ⅲ期-Ⅳ期牙周炎； （3）后牙区牙列完整至少至第一磨牙； （4）至少一颗后牙松动Ⅱ度-Ⅲ度，松动牙妨碍咀嚼功能或有不适，且受试者强烈要求保留患牙； （5）X线显示松动牙牙槽骨吸收至根长的1/2-2/3，未达根尖部； （6）有牙体牙髓病变的患牙先行充填及根管治疗； （7）受试者口腔卫生状况良好，依从性良好，可按时复诊。

Inclusion criteria

(1) Age 18-70; (2) Patients with stage Ⅲ/Ⅳ periodontitis; (3) The dentition of the posterior region is complete at least to the first molar; (4) ≥1 maxillary posterior tooth with mobility of degree Ⅱ-Ⅲ in combination with strong requests to retain the affected tooth/teeth; (5) X-ray showed that the alveolar bone of the loose tooth had been absorbed to 1/2-2/3 of the root length, but had not reached the root apex; (6)Treat the affected teeth with filling and root canal therapy if they have dental and pulp diseases; (7)Good oral hygiene and compliance.

排除标准：

（1）牙周炎处于进展期或牙周炎症没得到有效控制； （2）准备妊娠或处于妊娠期者； （3）后牙区存在严重牙体组织疾病或不良修复体的患者； （4）存在反??、对刃??等严重错??畸形情况的患者，或正畸治疗中的患者； （5）受试者不愿接受松动牙固定治疗； （6）吸烟者； （7）受试者依从性差，不能按时复诊，控制菌斑能力差； （8）患有严重系统性疾病或系统性疾病未控制（糖尿病、血液系统疾病、骨质疏松、类风湿、肿瘤等.

Exclusion criteria：

(1) Periodontitis is in the advanced stage or the periodontal inflammation is not effectively controlled; (2) Pregnant or lactating patients; (3) Serious dental tissue diseases, abutments of fixed bridges and removable local dentures in the posterior dental region; (4) Crossbite, scissors bite and edge-to-edge occlusion or undergoing orthodontic treatment; (5) The subjects did not want to undergo loose tooth fixation treatment; (6)Smoker; (7) Poor compliance or inability to follow up regularly; (8) Severe systemic disease or uncontrolled systemic disease (diabetes, blood system disease, osteoporosis, rheumatoid disease, tumor, etc.).

由专人对受试者进行随机分组。通过随机数字表为每一位受试者生成相应的随机数，将所得随机数除以组数“4”，按所得余数分别指定为C组、E1组、E2组、PT组：余数为“1”者分入C组，余数为“2”者分入E1组，余数为“3”者分入E2组，整除者分入PT组，并记录在案。在实施过程中不作任何限制、干预或调整。

The subjects were randomly grouped by specially-assigned person. The corresponding random number was generated for each subject through the random number table,and the obtained random number was divided by the group number "4", which was designated as group C, group E1, group E2 and group PT according to the obtained remainder: those with a remainder of "1" were divided into group C, those with a remainder of "2" were divided into group E1, those with a remainder of "3" were divided into group E2, and those with an integral division were divided into group PT, which was recorded. No restrictions, interventions or adjustments are made in the implementation process.
