研究目的:

本课题拟通过评估几种不同减重饮食对超重和肥胖高尿酸血症患者短期带来的影响,包括对血尿酸水平和痛风发作的影响评估,找到最佳减重饮食方式以避免减重初期血尿酸升高和痛风发作,并探讨减重早期尿酸水平升高和诱发痛风发作的影响因素,为后续的减重过程中高尿酸血症和痛风发作的预防提供干预靶点。  

Objectives of Study:

This project aims to find the best weight-loss diet to avoid elevated blood uric acid and gout attacks in the early stage of weight loss by evaluating the benefits and harms of several different weight-loss diets on overweight and obese hyperuricemia patients in the short term (one month before), including the evaluation of the impact on blood uric acid levels and gout attacks, and to explore the influencing factors of elevated uric acid levels and gout attacks in the early stages of weight loss, so as to provide intervention targets for the prevention of hyperuricemia and gout attacks in the subsequent weight loss process.

Description for medicine or protocol of treatment in detail:

 

纳入标准:

1)18-65周岁的男性; 2)诊断高尿酸血症者,指正常嘌呤饮食状态下,非同日两次 空腹检测血尿酸>420μmol/L(7mg/dl) 3)BMI≥25.0 kg/m2;

Inclusion criteria

1) Male aged 18-65; 2) Patients diagnosed with hyperuricemia refer to the fasting test of blood uric acid > 420 μmol/L (7 mg/dl) twice on the same day under normal purine diet 3)BMI≥25.0 kg/m2;

排除标准:

?1)入组前2周内使用可能影响尿酸的药物,包括:苯溴马隆、别嘌呤醇、非布司他等; 2)入组前2周内有痛风发作; 3)严重肝功能不全或肾脏疾病(AST 或 ALT>正常上限的 3 倍,或eGFR<30ml/min/1.73 m2); 4)存在影响糖脂代谢的其他疾病:糖尿病、甲状腺功能亢进症、甲状腺功能减退症、皮质醇增多症等; 5)如合并应用其他药物,需入组前三个月保持药物剂量稳定; 6)继发疾病或药物导致的肥胖; 7)患有严重的胃肠道疾病患者;

Exclusion criteria:

?1) Use of drugs that may affect uric acid within 2 weeks before enrollment, including: benzbromarone, allopurinol, febuxostat, etc.; 2) Gout attack within 2 weeks prior to enrollment; 3) severe hepatic insufficiency or renal disease (AST or ALT> 3 times the upper limit of normal, or eGFR <30ml/min/1.73 m2); 4) Other diseases that affect glucose and lipid metabolism: diabetes, hyperthyroidism, hypothyroidism, hypercortisolism, etc.; 5) If other drugs are used in combination, the drug dose should be kept stable for three months before enrollment; 6) obesity caused by secondary diseases or medications; 7) Patients with severe gastrointestinal diseases;
