研究目的：

通过对基层全科医生就2型糖尿病微血管并发症的知信行进行研究，探究基层全科医生相关知识水平和薄弱之处；了解其开展2型糖尿病微血管并发症社区筛查的态度，评价其社区筛查的行为，寻找基层全科医生态度和行为背后的原因，提出并实施改进策略；评价提升基层全科医生知信行对2型糖尿病微血管并发症社区早期筛查和干预的影响和效果。  

Objectives of Study：

To study the knowledge attitude and practice of primary care general practitioners(GPs) about microvascular complications of type 2 diabetes, to explore the level of knowledge and weaknesses of primary care GPs; to understand their attitudes towards community screening for microvascular complications of type 2 diabetes, to evaluate their behaviour towards community screening, to find out the reasons behind the attitudes and behaviours of primary care GPs, and to propose and implement strategies for improvement; and to evaluate the impact and effectiveness of enhancing the knowledge attitude and practice of primary care GPs on the early screening and intervention of microvascular complications of type 2 diabetes.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.基层全科医生：在社区卫生服务中心工作、有2型糖尿病签约服务对象的全科医生。 2.2型糖尿病患者：符合《中国2型糖尿病防治指南（2020年版）》中糖尿病诊断标准、能正常交流并配合检查的，年龄≥18岁的T2DM患者。

Inclusion criteria

1. Primary care GPs:GPs working in community health centers with type 2 diabetes contracted clients. 2. Patients with type 2 diabetes mellitus: patients with T2DM aged ≥18 years who meet the diagnostic criteria for diabetes mellitus in the Chinese Guidelines for the Prevention and Control of Type 2 Diabetes Mellitus (2020 Edition), who are able to communicate normally and cooperate with the examination.

排除标准：

1. 基层全科医生：签约服务对象中无2型糖尿病的基层全科医生。 2. 2型糖尿病患者：有严重躯体疾病，无法配合检查者；因精神疾病或认知障碍，无法正常交流者；妊娠者。

Exclusion criteria：

1. Primary care GPs: primary care GPs without type 2 diabetes patients among the contracted service recipients. 2. Patients with type 2 diabetes mellitus: those with serious physical illnesses who are unable to cooperate with the examination; those who are unable to communicate normally due to mental illness or cognitive impairment; and those who are pregnant.
