研究目的：

主要目标 (1) 基于首次移植的分析： 评估子宫内膜薄的 IVF-ET 患者在首次胚胎移植周期中新鲜胚胎移植与全胚冷冻策略的活产率，从而提供证据支持或反驳将子宫内膜薄作为取消新鲜胚胎移植的标准。 (2) 基于时间-事件的分析： 通过计算累计活产率，评估新鲜胚胎移植和完全胚胎冷冻在促进单次试管婴儿治疗实现活产方面的功效，同时将实现活产的时间作为时间组成部分。 次要目标 (1) 评估接受新鲜胚胎移植或全部冷冻策略（后续的冻融胚胎移植）患者的其他妊娠结局和新生儿结局。 (2）评估在子宫内膜薄的人群中，子宫内膜厚度的变化是否会改变两种策略对首次移植活产率和累积活产率的影响。 (3) 评估新鲜胚胎移植和全部冷冻策略对子宫内膜薄妇女中不同亚群的影响差异。  

Objectives of Study：

Primary objectives (1) First-transfer based analysis: To assess the incidence rate of live birth of fresh embryo transfer versus freeze-all strategy in the initial embryo transfer cycle for patients with a thin endometrium undergoing IVF-ET, thus providing evidence supporting or refuting the use of thin endometrium as a criterion for canceling fresh embryo transfers. (2) Time-to-event based analysis: To assess the efficacy of fresh embryo transfer and complete embryo freezing in facilitating the achievement of a live birth from a single IVF treatment by calculating the cumulative live birth rate, while considering the time to achieving live birth as a time component. Secondary objectives (1) To assess other pregnancy outcomes and neonatal outcomes of live births followed by the fresh embryo transfer or freeze-all strategy (frozen-thawed embryo transfer). (2) To assess whether variations in endometrial thickness alter the impact of two strategies on the live birth rate of the first transfer and cumulative live birth rate in a population with thin endometrium. (3) To assess the difference in the impact of fresh embryo transfer and freeze-all strategy in subgroups of women with a thin endometrium

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 年龄 ≥ 20 周岁，且 ≤ 50周岁的女性； 2. 患者在 IVF 周期中的子宫内膜测量值满足薄型子宫内膜诊断，定义为： HCG扳机日经超声引导下 B 超测量的子宫内膜厚度 < 8 mm；子宫内膜厚度定义为通过超声测量的子宫中矢状面上从一个子宫内膜-子宫肌层界面到另一个界面的最大距离。通常而言，HCG 日的内膜厚度会作为决定是否对患者进行新鲜胚胎移植的决定因素之一，因此进行重复测量，并取平均值以避免误差； 3. 患者在 IVF 周期取卵后，又至少接受了一次后续的胚胎移植（新鲜移植或冻融胚胎移植）。

Inclusion criteria

1.Women aged 20 to 50 years; 2.In an IVF cycle, if the endometrial thickness is diagnosed as thin, defined as an endometrial thickness < 8 mm measured via ultrasound on the HCG trigger day, the endometrial thickness is determined as the maximum distance from one interface between the endometrium and myometrium to the other in the sagittal plane of the uterus. The endometrial thickness on the HCG day is usually used to decide whether to proceed with a fresh embryo transfer. To minimize error, repeat measurements are taken and averaged; 3.The patient underwent at least one subsequent embryo transfer (either fresh or frozen) after egg retrieval in the IVF cycle.

排除标准：

研究排除了涉及以下内容的试管婴儿周期 ： (1) 植入前基因检测或诊断； (2) 使用冷冻卵母细胞/精子； (3) 捐献卵细胞周期； (4) 体外成熟周期； (5) 存在其他取消鲜胚移植的明确指征的周期，包括： ①子宫腔、子宫附件存在异常，如子宫内膜结构障碍/息肉/腺肌病/内异症/输卵管积 水/宫腔憩室； ②扳机日孕酮已经升高（定义为＞1.5 ng/ml）； ③存在高卵巢过度刺激（OHSS）风险（定义为获卵数＞=18 个） ④使用黄体期促排方案、高孕激素状态促排方案、克罗米芬促排方案的周期 (6) 没有取到卵母细胞、没有获得 2PN 子代细胞或在取卵后第三天没有可存活胚胎的周期也被排除在外。

Exclusion criteria：

The study excluded IVF cycles involving: (1) Pre-implantation genetic testing or diagnosis; (2) The use of frozen oocytes/sperm; (3) Donor oocyte cycles; (4) In vitro maturation cycles; (5) There are cycles with clear indications for canceling fresh embryo transfer, including: ①Abnormalities in the uterine cavity or adnexa, such as endometrial structural disorders, polyps, adenomyosis, endometriosis, hydrosalpinx, or uterine diverticulum; ②Elevated progesterone on the trigger day (defined as > 1.5 ng/ml); ③Risk of severe ovarian hyperstimulation syndrome (OHSS) (defined as ≥ 18 oocytes retrieved); ④Cycles using luteal phase stimulation, high progesterone stimulation, or clomiphene stimulation protocols. (6) Cycles where no oocytes were retrieved, no 2PN zygotes were obtained, or no viable embryos were available on day three post-retrieval were also excluded.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

1. 数据处理和记录保存 患者的保密权至关重要。参与中心提供的所有电子数据库都将对患者信息进行匿名处理；因此，最终合并的数据库将不包含任何可识别的信息。任何研究人员都无法获得参与本研究的患者的完整身份信息。收集到的数据将仅用于本研究目的。 2. 数据保密和时限 数据将由参与中心的研究人员提供给山东中医药大学附属医院，并在那里进行整合和安全保护。韩乔松和郭子珍负责监督数据收集和合并数据库的建立，韩乔松和其他统计分析师负责数据清理和统计分析。 这项研究的时间安排--从接收所有数据、组装和清理数据库到统计分析和预期的初步结果--估计为六个月。截至试验注册前，数据库组装已经完成，数据清理正在进行，统计分析尚未开始。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

1. DATA HANDLING AND RECORD KEEPING The Patient’s right to confidentiality is paramount. All electronic databases provided by the participating centers will anonymize patient information; consequently, the final merged database will contain no identifiable information. No researcher will have access to the complete identity details of patients involved in this study. The collected data will be exclusively used for the purposes of this study. 2. DATA CONFIDENTIALITY AND TIMELINE Data will be supplied to the Affiliated Hospital of Shandong University of Traditional Chinese Medicine by investigators from the participating centers, where it will be consolidated and securely protected. Qiao-Song Han and Zi-Zhen Guo oversee the collection of data and the establishment of the merged database, while Qiao-Song Han and additional statistical analysts will conduct the data cleaning and statistical analyses. The timeline of the study—from the receipt of all data and the assembly and cleansing of the database to the statistical analyses and the anticipated initial results—is estimated at six months. Up to the trial registration, the database assembly is complete, data cleansing is underway, and statistical analysis has not yet commenced.
