研究目的：

本研究拟通过“医院-社区”联动管理方案对结直肠息肉镜下切除术后社区居民进行健康管理，旨在改善居民结肠镜检查依从性，提高其结直肠癌防治知识知晓率和健康行为水平，降低城市结直肠癌发病率，提高生存率；为政府部门癌症防控决策提供参考依据，也为进一步探索结直肠息肉发病机制提供研究基础。  

Objectives of Study：

In this study, the "hospital-community" linkage management plan was used to conduct health management for community residents after colorectal polypectomy, aiming to improve residents' colonoscopy compliance, increase their awareness rate of colorectal cancer prevention knowledge and healthy behaviors, Reduce the incidence of colorectal cancer in urban areas and improve the survival rate; provide a reference for government departments to make decisions on cancer prevention and control, and provide a research basis for further exploration of the pathogenesis of colorectal polyps.

纳入标准：

（1）户籍为南昌市市区的常住居民，年龄为45-74周岁； （2）结肠镜下切除并证实为结直肠息肉者； （3）无严重器官功能障碍和精神疾病者。

Inclusion criteria

(1) Permanent residents with household registration in the urban area of Nanchang, with an age range of 45-74 years. (2) People with colonoscopic resection and confirmed colorectal polyps. (3) People without serious organ dysfunction and mental illness.

排除标准：

（1）已确诊恶性肿瘤，或合并炎症性肠病及其他需要住院治疗的疾病者； （2）既往曾患脑卒中、心肌梗死等疾病者； （3）妊娠或哺乳期妇女； （4）入选本研究前三个月内参与其他研究试验者。

Exclusion criteria：

(1) People who have been diagnosed with malignancy, or combined with inflammatory bowel disease and other diseases requiring hospitalization. (2) People with previous stroke, myocardial infarction and other diseases. (3) Pregnant or lactating women. (4) People who participated in other research trials within three months prior to enrollment in this study.

In order to avoid information bias and confounding bias that may be caused by communication between residents and medical staff who follow up regularly at the same community health centre, this study used the whole cluster random grouping method to randomly group the 82 communities where the study subjects were located into an intervention group and a control group.
