研究目的:

(1)基于可穿戴设备与生物雷达技术对心衰患者进行无间断远程监测,从监测的BCG和SCG信号中提取有效生理参数或指标; (2)利用可穿戴设备与生物雷达技术采集的各项生理参数或指标,结合患者基线特征以及血液标志物,研发心衰加重的预警模型,并对模型进行内部和外部验证及优化,为患者管理提供可行方向和相关数据。  

Objectives of Study:

1. With the application of wearable devices and biological radar equipment to acquire continuous real-time BCG and SCG signals, and then extract effective physiological indicators; 2. Combined with the characteristics and serum biomarkers at baseline, we used physiological indicators monitored from wearable devices and biological radar equipment to build the predictive model of heart failure patients with acute exacerbation and then to conduct internal and external validation and model optimization.

纳入标准:

1. 年龄在18岁及以上 2. 深圳市常住居民(过去12月在深圳居住超过6月者) 3. 根据《国家心力衰竭指南2023》(完整版)诊断为NYHA功能III-IV级的慢性心力衰竭患者 4. 表示能够接受并遵循研究的后续要求和完成每日监测任务 5. 签署知情同意书

Inclusion criteria

1. Aged >= 18 years; 2. Permanent Shenzhen residents (who have lived in Shenzhen for at least six months in the past one year); 3. Diagnosed as heart failure with III-IV NYHA according to National Heart Failure Guideline 2023 ; 4. Willing to accept and comply with follow-up requirements and daily monitoring; 5. Signed informed consent.

排除标准:

1. 在过去一个月内发生过重大心血管事件(如心肌梗死、心脏手术、中风) 2. 计划或可能在基线入组后6个月内接受心脏移植或接受心室辅助装置的受试者 3. 起搏器依赖患者(依赖起搏器频率占比≥40%) 4. 因病情严重、精神异常或语言交流困难等原因无法正常完成研究者 5. 基线住院期间死亡者

Exclusion criteria:

1. Had the major cardiovascular events one month prior to recruitment, such as myocardial infarction, heart surgery, stroke; 2. Had the plan or the possibility to receive heart transplantation or a ventricular assist device; 3. Pacemaker-dependent patients (pacemaker-dependent frequency ≥40%); 4. Those who are unable to conduct the questionnaire survey due to serious illness, mental disorder, or difficulty in communication; 5. Dead during hospitalization at baseline.

数据采集和管理(说明:数据采集和管理由两部分组成,一为病例记录表(Case Record Form, CRF),二为电子采集和管理系统(Electronic Data Capture, EDC),如ResMan即为一种基于互联网的EDC:

1. 基线和随访问卷中的信息直接由面对面或电话访谈获得; 2. 实时心率、心律及心律变异性、呼吸频率和模式、夜间咳嗽、睡眠模式等信息通过手环和毫米波雷达设备进行监测; 3. 其他信息从病历系统中获取到EDC系统中; 4. 对生物样本进行高通量测序,数据保存在中国医学科学院阜外医院深圳医院本地服务器中。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

1. The variables in the baseline and follow-up questionnaires were generated directly from face-to-face or telephone interviews; 2. Real-time heart rate, heart rate and heart rate variability, respiratory rate and patterns, nighttime cough, and sleep patterns are monitored through bracelets and millimeter-wave radar devices; 3. Other information is obtained from the medical records, and will be exported or typed into the EDC system; 4. High-throughput sequencing data from biological samples will be stored in the local servers of Fuwai Hospital Chinese Academy of Medical Sciences, Shenzhen.
