研究目的：

本研究通过前瞻性随机对照试验，对比结肠水疗仪灌肠和再次口服结肠清洁剂两种不同补救措施的对肠道清洁程度的改善效果、对结肠镜检查结果的影响及患者的耐受性和满意度，探讨肠道准备不佳时在不取消当天检查的情况下补救措施的优选方案。  

Objectives of Study：

This study involved a prospective randomized controlled trial designed to evaluate the efficacy of two different therapeutic modalities, a colon hydrotherapy device enema and re-administration of oral colon cleanser, in improving bowel cleansing, colonoscopy outcomes, and patients' tolerability and satisfaction. The trial also aimed to identify preferred options for bowel preparation in cases of poor preparation without the need to postpone the examination.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）我院门诊或住院预约结肠镜检查患者，年龄为18-80岁； （2）按原定肠道准备方案进行肠道准备后，最后一次肛门排泄物（患者或家属提供马桶内排泄物照片，由一名专业医护人员核实评定）不符合清水样便或者淡黄色透明液体，判定为肠道准备不充分。或者在行普通电子结肠镜中依据波士顿量表＜6分，或者3段结肠中任意一段评分＜2分，判断为肠道准备不合格者； （3）患者自愿签署知情同意书，愿意遵循研究指示，有能力完成整个研究程序。

Inclusion criteria

(1)Patients with proposed colonoscopy in our hospital, aged 18-80 years old; (2)Bowel preparation according to the original bowel preparation programme, the last anal excretion does not meet the watery stool or yellowish transparent liquid, judged as inadequate bowel preparation (judged as very poor bowel preparation: faecal water is brown, containing a large amount of waste; slightly poor bowel preparation: faecal water is dark yellow, containing a small amount of faecal sludge); or in the performance of ordinary electronic colonoscopy based on the Boston scale <6 points, or any of the three segments of the colon <2 points, judged to be inadequate bowel preparation; (3)Patients voluntarily sign an informed consent form, follow the study instructions and can co-operate with the entire study procedure.

排除标准：

（1）存在严重心脑血管疾病、心肺功能不全、休克、腹主动脉瘤、急性腹膜炎、肠穿孔、肠梗阻、中毒性巨结肠疾病； （2）肛门、直肠有严重化脓性炎症或者疼痛性病灶，如肛周脓肿、肛裂； （3）患精神疾病或者不能配合者； （4）活性的消化道出血患者； （5）妊娠期女性、月经期女性； （6）患有或疑似炎症性肠病患者； （7）结直肠手术史导致正常生理解剖结构异常； （8）聚乙二醇过敏者； （9）研究者认为不适合加入本研究者的其他情况。

Exclusion criteria：

(1) Presence of severe cardiovascular or cerebrovascular disease, cardiopulmonary insufficiency, shock, abdominal aortic aneurysm, acute peritonitis, intestinal perforation, intestinal obstruction, toxic megacolon disease; (2) Presence of severe purulent inflammation or painful lesions in the anus or rectum, such as perianal abscess, anal fissure; (3) Mental illness or inability to co-operate; (4) Patients with active gastrointestinal bleeding; (5) Pregnant women and menstruating women; (6) Patients with or suspected of inflammatory bowel disease; (7) Individuals who have undergone colorectal surgery resulting in abnormal normal physiological anatomy; (8) Individuals with a known allergy to polyethylene glycol; (9) Other conditions that the investigator deems unsuitable for enrolment in this study.

研究人员进行以下操作（1）患者按进入试验时间先后编码，区组长度定义为4，对每个区组内的4个人进行随机化分配，直到2个人被随机化分配到其中一组，从后其余受试者自动分配到另一组。（2）每个区组内的随机化分配，获取随机数字，从随机数字表中任意一个数开始,沿同一方向顺序获取一个随机数字，随机数除以组数求余数，可整除进入结肠水疗仪灌肠组（灌肠组），余数为1进入再次口服结肠清洁剂（口服组）。

Randomization Procedure (please state who generates the random number sequence and by what method)：

The researchers performed the following operations (1) patients were coded according to the time of entry into the trial successively, the length of the zone group was defined as 4, and the randomisation of the allocation of the 4 individuals within each zone group was carried out until 2 individuals were randomised to one of the groups, and from then on the rest of the subjects were automatically allocated to the other group. (2) Randomisation within each group, obtaining a random number, starting from any number in the random number table, obtaining a random number in the same direction, dividing the random number by the number of groups to find the remainder, which can be rounded up to enter the enema group of the colon hydrotherapy instrument (enema group), and the remainder of 1 to enter the oral colon cleansing agent again (oral group).
