研究目的：

本研究对比低频重复经颅磁刺激（repetitive transcranial magnetic stimulation,rTMS）与爆发式经颅磁刺激（theta burst stimulation,TBS）的两种治疗模式（间歇性TBS和持续性TBS）对非流利性失语患者的语言功能、听理解及生活质量的改善情况，以探究TBS对卒中后失语症的康复疗效。  

Objectives of Study：

This study compared repetitive transcranial magnetic stimulation (rTMS) with theta burst Two treatment modalities (intermittent TBS and continuous TBS) improve speech function, hearing comprehension, and quality of life in patients with nonfluent aphasia to investigate the efficacy of TBS in the rehabilitation of post-stroke aphasia.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①符合1995年全国第四届脑血管病学术会议制定的脑卒中诊断标准， ②根据西方失语症成套测验（western aphasia battery,WAB)诊断为非流利性失语患者， ③首次发病， ④病灶均在左侧大脑半球， ⑤年龄大于18岁， ⑥发病前语言功能正常， ⑦小学及以上文化程度， ⑧母语为汉语， ⑨右利手， ⑩患者及家属知晓本次实验内容，并签署知情同意书。

Inclusion criteria

① It meets the diagnostic criteria for stroke formulated by the Fourth National Conference on Cerebrovascular Diseases in 1995. (2) Patients with non-fluent aphasia are diagnosed according to the western aphasia battery (WAB), ③ First onset, ④ The lesions were all in the left cerebral hemisphere. ⑤ over 18 years old, ⑥ Normal language function before onset, ⑦ Primary school education and above, ⑧ Native Chinese speaker, ⑨ Right-handed, ⑩ Patients and their families know the content of this experiment and sign the informed consent.

排除标准：

①合并意识障碍，不能配合检查和治疗者， ②合并视力和听力障碍，可能会干扰评价及治疗者， ③本次发病之前已有语言障碍者或本次合并严重构音障碍者， ④合并癫痫或其它疾病致无法配合治疗者， ⑤并发其他神经系统变性疾病，如痴呆、帕金森等所致的言语障碍者， ⑥合并恶性高血压， ⑦合并恶性肿瘤的患者， ⑧经实验室检验、检查发现合并有严重心、肺、肝、肾等系统性疾病且常规用药无法控制的患者， ⑨存在其他检查异常研究者判断不适合参与此试验的患者， ⑩体内有金属异物或其他植入性电子装置， ?存在严重抑郁、焦虑状态，或诊断有其他精神疾病以致无法配合完成试验的患者， ?正在参加其他临床研究试验者。

Exclusion criteria：

① Patients with combined consciousness disorder who cannot cooperate with examination and treatment, (2) Patients with combined visual and hearing impairment that may interfere with evaluation and treatment, ③ Patients with speech disorder before the onset of the disease or with severe dysarthria, ④ Unable to cooperate with treatment due to epilepsy or other diseases, ⑤ Speech disorders caused by other neurodegenerative diseases, such as dementia and Parkinson's disease, ⑥ With malignant hypertension, ⑦ Patients with malignant tumors, Patients with severe systemic diseases of the heart, lung, liver, kidney and other diseases that cannot be controlled by conventional drugs have been found through laboratory tests and examinations. ⑨ Patients with other test abnormalities determined by the investigator to be unsuitable for participation in this study, ⑩ Metal foreign bodies or other implanted electronic devices in the body, ? Patients with severe depression, anxiety or diagnosed with other mental illness who are unable to cooperate with the completion of the test, ? Still participating in other clinical research trials.

由一名不参与评价指标和治疗过程的治疗师通过随机数字表法产生随机序列， ①编号：将研究对象按入院时间进行编号,1-66。 ②取随机数：从随机数字表（参考《医学统计学》第四版中的附表15）中任意一行任一列开始，一次读取66个相同位数的数字作为一个随机数录于编号下。 ③排序：将全部随机数从小到大编序号，将每个随机数字对应的序号记在表中。 ④分组：规定序号1-22为A组，23-44为B组，45-66为C组。

A therapist who is not involved in the evaluation and treatment process generates a random sequence using a random number table method, ① Number: The subjects were numbered according to the time of admission,1-66. (2) Take random numbers: Starting from any row or column in the random number table (refer to Schedule 15 in the fourth edition of Medical Statistics), read 66 numbers of the same number at a time as a random number recorded under the number. ③ Sorting: all random numbers are numbered from small to large, and the corresponding serial number of each random number is recorded in the table. ④ Grouping: the serial number 1-22 is group A, 23-44 is group B, and 45-66 is group C.

Blinding：

① The random number is generated by a special person, and the experimenter and the subject are unaware of it in advance. (2) Baseline and outcome measures of patients' language ability and quality of life were assessed by the same speech therapist who did not know the assignment results and did not participate in the treatment process. The analysis and processing of the data were carried out by speech therapists who were not aware of the assignment results and did not participate in the treatment process. (4) In the course of the experiment, the patient and the speech therapist could not be blinded due to the involvement of speech therapy.
