研究目的：

本研究通过麻醉医师对中、老年肺癌吸烟患者术前实施戒烟干预，评估宣教及右美托咪定对患者免疫功能、戒烟情况、术后并发症及长期预后影响。 旨在强调麻醉医师参与患者术前戒烟的重要性，将戒烟质量与肺癌患者免疫功能作为研究重点，以期为中、老年吸烟肺癌患者戒烟提供更合适的药物选择，提高患者的生活质量，延长患者的生存时间  

Objectives of Study：

This study evaluated the effects of education and dexmedetomidine on immune function, smoking cessation, postoperative complications and long-term prognosis of middle-aged and elderly patients with lung cancer through preoperative smoking cessation intervention by anesthesiologists. This study aims to emphasize the importance of anesthesiologists' participation in preoperative smoking cessation, and take the quality of smoking cessation and immune function of lung cancer patients as the research focus, in order to provide more suitable drug choices for middle-aged and elderly patients with lung cancer smoking to quit smoking, improve the quality of life of patients and extend the survival time of patients

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1 、年龄 40-75岁 吸烟患者；2 、ASAI-II 级；3 、影像学提示一侧肺叶单个结节或肿块；4 、择期行胸腔镜肺癌根治术。

Inclusion criteria

1. Smoking patients aged 40-75 years old; 2. ASAI-II level; 3. Imaging shows a single nodule or mass in one lung lobe; 4. Thoracoscopic radical resection of lung cancer was performed.

排除标准：

1 、酗酒及阿片类滥用及过敏史；2 、拒绝参与者；3 、 曾经或正在 使用尼古丁替代治疗等戒烟干预；4 、行急诊手术；5 、窦性心动过缓者。

Exclusion criteria：

1. History of alcoholism and opioid abuse and allergy; 2. Refuse participants; 3. Have used or are using nicotine replacement therapy and other smoking cessation interventions; 4, emergency surgery; 5. Sinus bradycardia.

手术前一天与患者在医师办公室进行交谈，对患者说一句希望能戒烟，并用图文并茂的戒烟宣传册进行面对面戒烟健康教育20分钟。患者入手术室后常规监测生命体征，开通静脉后泵注右美托咪定0.5ug/kg，泵注时间15min，泵注结束后开始麻醉诱导；术后按照0.05ug/kg/h把右美托咪定加入术后镇疼泵，术后镇疼72h。

The day before the operation, he talked with the patient at the doctor's office, told the patient that he hoped to quit smoking, and conducted face-to-face health education on smoking cessation with an illustrated smoking cessation brochure for 20 minutes. After entering the operating room, the patient's vital signs were routinely monitored. After intravenous opening, 0.5ug/kg dexmedetomidine was injected with the pump for 15min. Anesthesia induction began after the pump was finished. After operation, dexmedetomidine was added to the postoperative pain relief pump according to 0.05ug/kg/h for 72h.
