研究目的：

本研究旨在运用康复治疗规范体系(Rehabilitation Treatment Specification System, RTSS)理论框架，结合ICF和世界卫生组织《社区康复指南》的架构和理论指导，构建适应中国老年社区的医体结合服务体系以促进社区健康。  

Objectives of Study：

The purpose of this study was to apply the theoretical framework of Rehabilitation Treatment Specification System (RTSS) and the framework and theoretical guidance of ICF and who community Rehabilitation guidelines, to construct a medical-physical integration service system for the elderly community in China in order to promote community health.

纳入标准：

1.具有社区康复需求的老年人群；

Inclusion criteria

1.Elderly people with community rehabilitation needs;

排除标准：

（1）生理或心理状况不稳定者； （2）无法正确理解并签署知情同意书者； （3）正在接受其他可能影响研究结果的治疗者； （4）无法遵守研究协议或要求、中途退出的治疗者。

Exclusion criteria：

(1) Those with unstable physical or psychological conditions; (2) Those who are unable to properly understand and sign the informed consent form; (3) Those who are receiving other treatments that may affect the results of the study; (4) Those who are unable to comply with the study protocol or requirements and withdraw from the treatment halfway.
