针对胸部CT提示中肺野磨玻璃为主cT1N0患者，探索肺段切除的手术短期结局与长期生存获益，初步揭示肺段切除的安全性及有效性。  

Objectives of Study：

For patients with ground-glass opacity mainly in the lung field indicated by chest CT, explore the short-term outcomes of segmental resection surgery and the long-term survival benefits, and preliminarily reveal the safety and effectiveness of segmental resection

纳入标准：

1，胸部CT满足以下所有标准： a. 怀疑肺癌（排除中叶） b. N0 c. T <=3cm; C/T＜＝ 0.5 d. 三维重建肿瘤中心位于肺野中1/3，即第二隆突到肿瘤中心距离/第二隆突到胸膜表面距离小于2/3 e. 三维重建上规划切缘：保证切缘大于2cm或肿瘤直径 2. 多余的结节数少于2个, 多余的结节满足以下： a. T<=2cm; C/T<0.25 并位于肺野外1/3. b.纯 GGO 且<=1cm 3. R0切除 4. ECOG 评分 0或1 5. 18-75岁 6. 非同侧开胸手术（诊断性胸腔镜可以） 7. 对侧未做过肺切除（楔形除外） 8. 未做过放化疗 9. 肺功能 FEV1.0>=1L 10. 签署同意 11. 血检达标

Inclusion criteria

1. Chest CT meets all of the following criteria: a. Suspected lung cancer (excluding the middle lobe) b. N0 c. T ≤ 3cm; C/T ≤ 0.5 d. Three-dimensional reconstruction shows the tumor center is located in the middle one-third of the lung field, that is, the distance from the second carina to the tumor center / the distance from the second carina to the pleural surface is less than 2/3 e. Planned resection margin on three-dimensional reconstruction: ensuring the margin is greater than 2cm or equal to the tumor diameter 2. The number of additional nodules is less than 2, and the additional nodules meet the following: a. T ≤ 2cm; C/T < 0.25 and located in the outer one-third of the lung field b. Pure ground-glass opacity (GGO) and ≤ 1cm 3. R0 resection

排除标准：

1. 最近5年内其它恶性肿瘤病史，原位或者粘膜内肿瘤除外 2. 合并间质性肺炎，肺纤维化，重度COPD 3. 任何不稳定的全身疾病：难以控制的高血压，不稳定性心绞痛，充血性心力衰竭，半年内心肌梗塞，严重心律失常，肝脏、肾脏、或代谢性疾病 4. 怀孕，拟怀孕，或者哺乳期 5. 精神疾病 6. 难以控制的感染 7. 研究者认为其它不适合入组的情况

Exclusion criteria：

1. History of other malignant tumors within the last 5 years, except for in situ or mucosal tumors. 2. Concurrent interstitial pneumonia, pulmonary fibrosis, or severe COPD (Chronic Obstructive Pulmonary Disease). 3. Any unstable systemic disease: uncontrollable hypertension, unstable angina, congestive heart failure, myocardial infarction within the past six months, severe arrhythmia, liver, kidney, or metabolic disorders. 4. Pregnancy, planning to become pregnant, or breastfeeding. 5. Mental illness. 6. Infections that are difficult to control. 7. Any other conditions deemed unsuitable for enrollment by the investigator.

本研究为单臂多中心前瞻性观察性研究，所有检验检查及手术方案均为常规诊疗方案。所有患者术前均进行三维重建，在三维图像中，肿瘤中心位置位于中肺野（第二隆突至肿瘤中心距离/第二隆突至胸膜表面距离满足：大于1/3，小于2/3），术中采用以肺段切除为基础的亚肺叶切除（包括肺段切除，扩大肺段切除，肺段切除+亚段切除），系统性淋巴结清扫或取样，术中测量切缘2cm以上或者大于肿瘤直径，术中送检可疑淋巴结（直径最大淋巴结），如果淋巴结阳性或者切缘不满足，则改为肺叶切除。

Intervention：

This study is a single-arm, multicenter, prospective, observational study, where all tests, examinations, and surgical protocols are part of the standard diagnostic and treatment plan. All patients undergo three-dimensional reconstruction before surgery. In the three-dimensional images, the tumor center is located in the middle lung field (the distance from the second carina to the tumor center / the distance from the second carina to the pleural surface meets: greater than 1/3, less than 2/3). During surgery, sublobar resection based on segmental resection is performed (including segmentectomy, extended segmentectomy, segmentectomy + sub-segmentectomy), systematic lymph node dissection or sampling, and the resection margin is measured to be more than 2cm or greater than the tumor diameter during surgery. Suspicious lymph nodes (the largest in diameter) are sent for intraoperative examination; if the lymph nodes are positive or the margin is not met, the procedure is changed to lobectomy.
