研究目的：

当前我国肠造口患者数量与日俱增，且肠造口护理难度系数大、身体形象的改变使患者经受着身心双重打击，如何提高肠造口患者的生活质量一直备受医护人员关注。本研究以支持性照护理论为框架创新性构建成人肠造口患者居家照护方案，其成果对满足患者多层次照护需求、改善患者生活质量具有非常重要的实践意义。  

Objectives of Study：

At present, the number of enterostomy patients in China is increasing day by day, and the difficulty coefficient of enterostomy nursing is large, and the change of body image makes patients suffer from both physical and mental blows. How to improve the quality of life of enterostomy patients has always been concerned by medical staff. Based on the theory of supportive care, this study innovatively constructs a home care program for adult patients with enterostomy. The results have very important practical significance for meeting the multi-level care needs of patients and improving the quality of life of patients.

纳入标准：

①通过肠造口术进行治疗的永久性肠造口患者；②年龄≥18岁；③患者或家属能熟练使用智能手机、电脑等设备；④知情同意，自愿参与本次访谈。

Inclusion criteria

1. Patients with permanent enterostomy treated by enterostomy ; 2. age ≥ 18 years old ; 3 .patients or their families can skillfully use smart phones, computers and other devices ; 4 .Informed consent, voluntary participation in this interview.

排除标准：

①有明显的意识障碍、智力障碍导致无法正常沟通者；②无法配合或中途退出者。

Exclusion criteria：

1 .patients with obvious disturbance of consciousness and mental retardation leading to abnormal communication ; 2 .those who could not cooperate or quit halfway

(1)常规造口护理措施：在患者出院前一天加入“造口之家”微信群，出院后以电话或者网络随访的方式询问患者的造口护理情况、造口及周围皮肤情况等；出院指导内容包括指包括患者疾病知识等信息支持、照护技能指导（介绍不同造口袋种类和辅助用品的特点及储存方法、指导患者选用合适的造口护理用品、演示及讲解肠造口袋更换步骤及注意事项，鼓励患者家属录像，便于以后学习，讲述并发症的观察等造口相关护理知识）、心理疏导、社会支持等。 (2)出院前指导：出院前对患者进行居家生活指导，用药指导等内容，告知患者复查时间，指导第4周、12周后来院复查，告知患者科室电话，如有问题可随时电话咨询。 (3)出院后随访：出院后患者每周通过微信向研究者本人发送造口图片，监测造口并发症情况。

( 1 ) Routine stoma care measures : join the ' stoma home ' WeChat group the day before the patient is discharged from the hospital, and ask the patient 's stoma care, stoma and surrounding skin conditions by telephone or network follow-up after discharge ; the content of discharge guidance includes information support including patients ' disease knowledge, nursing skills guidance ( introducing the characteristics and storage methods of different types of ostomy bags and auxiliary supplies, guiding patients to choose appropriate ostomy nursing supplies, demonstrating and explaining the steps and precautions of ostomy bag replacement, encouraging family members of patients to videotape, so as to facilitate future learning, telling the observation of complications and other ostomy-related nursing knowledge ), psychological counseling, social support, etc. ( 2 ) Guidance before discharge : Before discharge, patients were given home life guidance, medication guidance, etc., to inform the patient 's review time, to guide the hospital review after 4 weeks and 12 weeks, to inform the patient 's department telephone, if there is a problem, you can call for advice at any time. ( 3 ) Follow-up after discharge : After discharge, the patients sent stoma pictures to the researchers themselves through WeChat every week to monitor the complications of stoma.

①在患者出院后，根据造口护理时间进展，通过信息平台推送相关内容，平均指导患者时间为15~20min/次。此外，对患者进行一对一个性化指导，内容包括：生理、心理、情感、信息、社会、精神、实际需求。 ②门诊复诊，包括3个部分：通过询问《造口患者生活质量量表》《造口患者社会心理适应量表》《造口自我护理量表-早期版》中主要条目评估患者情况，根据门诊复诊情况、患者居家造口护理出现的问题和严重程度提供具体干预措施和合适的转诊建议。具体时间为出院后4周和出院后12周，特殊患者根据病情需要增加次数，做到随时掌握患者情况，随时提供咨询。
