研究目的：

1.构建住院T2DM患者低血糖风险分层预防管理方案，为临床T2DM患者低血糖预防提供科学有效的管理方法。 2.验证住院T2DM患者低血糖风险分层预防管理方案对低血糖发生率等事件的影响。  

Objectives of Study：

1. Construct a risk stratified prevention and management scheme for hypoglycemia in hospitalized T2DM patients, and provide a scientific and effective management method for the prevention of hypoglycemia in clinical T2DM patients. 2. To verify the effect of risk stratified prevention and management of hypoglycemia in hospitalized T2DM patients on the incidence of hypoglycemia and other events.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1、符合1999年WHO T2DM诊断标准的住院患者； 2、年龄≥18岁； 3、自愿参与，且签署知情同意书。

Inclusion criteria

1 Hospitalized patients WHO met the 1999 WHO diagnostic criteria for T2DM; 2 Age ≥18 years old; 3 Voluntary participation and signed informed consent.

排除标准：

1、语言沟通障碍、精神行为异常者； 2、合并严重脏器功能障碍者； 3、入院24h内发生低血糖的患者； 4、认知障碍且无家属陪同的患者； 5、近期或正在参加其他类似研究的患者；

Exclusion criteria：

1.language communication barriers, mental behavior abnormalities; 2.Patients with severe organ dysfunction; 3.Patients with hypoglycemia within 24 hours of admission; 4.Patients with cognitive impairment and no accompanying family members; 5.Patients who have recently or are currently participating in other similar studies;

在对照组干预基础上采用《住院T2DM患者低血糖风险分层预防管理方案》进行干预。使用低血糖风险模型于患者入院后评估患者的低血糖风险等级，随后根据患者风险等级进行对应护理措施。
