研究目的：

结合智能信息技术，研制一项便于基层医疗人员开展老年跌倒预防工作的综合干预服务包，并评估该干预服务包的可行性和效果。  

Objectives of Study：

To develop a comprehensive intervention package which is convenient for community health care providers to carry out elderly fall prevention, and evaluate the feasibility and effectiveness of the intervention package, combined with artificial intelligence Iinformation technology.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）老年人≥65周岁；（2）老年人为所选研究现场的常住人口（在该地区居住半年以上），且近 1 年内没有搬迁计划；；（3）同意参加研究；（4）老年人能够回答调查人员的问题，或其日常照护者能够回答调查人员的问题、熟知老年人的基本情况且愿意协助其参与项目；（5）老年人具有中文阅读能力，或其日常照护者具有中文阅读能力且愿意协助其参与项目；（6）老年人有手机，或其日常照护者有手机且愿意协助其参与后续随访调查和干预。

Inclusion criteria

(1) ≥65 years old; (2) Permanent population of the selected study site (residing in the area for more than half a year) and having no relocation plans within the past year; (3) Consent to participate in the study; (4) The elderly are able to answer the investigators' questions, or their daily caregivers are able to answer the investigators' questions and are familiar with the elderly's basic situation and are willing to assist them in participating in the project; (5) The elderly have Chinese reading ability, or their daily caregivers have Chinese reading ability and are willing to assist them in participating in the project; (6) The elderly have mobile phones, or their daily caregivers have mobile phones and are willing to assist them in follow-up investigations and interventions.

排除标准：

（1）老年人长期卧床（因疾病或虚弱丧失自主活动能力和自理能力，已持续卧床 6 个月及以上）；（2）老年人患有可能影响其参与研究的不稳定或晚期的疾病；（3）老年人同期参加了其他跌倒干预项目。

Exclusion criteria：

(1) stay in bed for a long time (due to illness or weakness, losing the ability to move independently and take care of oneself, and having been bedridden continuously for 6 months or more); (2) The elderly have an unstable or terminal medical condition that may affect their participation in the study; (3) Participated in other fall intervention programs during the same period.

本研究采用分层多阶段整群随机的方法进行抽样。整个随机化过程采用SAS程序实现，由一名研究人员独立完成。首先，从长沙市和邵阳市城区中各随机抽取2个街道，总共选取4个街道。然后，在每个街道中，从老年人人数大于100人的社区中随机选择6个社区，总共抽取24个社区参与研究。将不同街道的社区随机分配到干预组和对照组，使得干预组和对照组各包含12个社区。同样，在农村地区抽取24个村庄参与研究，将不同乡镇的村庄随机分配到干预组和对照组，使得干预组和对照组各包含12个村庄。

Randomization Procedure (please state who generates the random number sequence and by what method)：

Stratified multistage cluster random sampling method was adopted. Randomization procedure will be performed by SAS software and completed by a reseacher. Firstly, 2 streets were randomly selected from Changsha city and Shaoyang City, and a total of 4 streets were selected. Then, in each street, 6 communities were randomly selected from the communities with more than 100 elderly people, and a total of 24 communities were selected to participate in the study. The communities of different streets were randomly assigned to the intervention group and the control group, so that the intervention group and the control group each contained 12 communities. Similarly, 24 villages in rural areas were selected to participate in the study, and villages in different townships were randomly assigned to the intervention group and the control group, so that the intervention group and the control group each contained 12 villages.
