本研究拟基于特定区域老年人群，探讨人口老龄化背景下季节性流感疫苗“接力种”激励策略的应用效果；通过三臂整群随机对照试验，评估“接力种”激励模式下老年人群的季节性流感疫苗接种情况，揭示不同创新激励策略的促进效果；通过进一步综合分析对比老年人群的流感疫苗接种意愿、流感疫苗接种率、以及流感疫苗接种成本效益比，探究促进老年人群接种季节性流感疫苗的最佳激励策略；为疫苗预防接种规划、疫苗强化接种、以及老龄化社会的健康促进，提供科学理论与技术支持。  

Objectives of Study：

This study aims to explore the effectiveness of incentive strategies for seasonal influenza vaccine "pay it forward" in the elderly population in specific regions against the backdrop of population aging. Through a three-arm cluster randomized controlled trial, we will assess the vaccination status of the elderly population under the "pay it forward" incentive model, and reveal the promotion effects of different innovative incentive strategies. Furthermore, through comprehensive analysis and comparison of the influenza vaccine acceptance, vaccination rate, and cost-effectiveness ratio, we aim to explore the optimal incentive strategy to promote seasonal influenza vaccine uptake among the elderly population. This study intends to provide scientific theories and technological support for vaccine prevention planning, vaccine booster vaccination, and health promotion in aging societies.

纳入标准：

① 年龄≥60岁的老年人； ②一年内未接种过季节性流感疫苗。

Inclusion criteria

①Elderly individuals aged 60 years and above. ②Individuals who have not received seasonal influenza vaccination within the past year.

排除标准：

① 患有急性疾病、严重慢性疾病或慢性疾病的急性发作期以及发热患者； ②有流感疫苗及相关成分过敏史； ③ 有医师认为不适合接种流感疫苗的其他情况。

Exclusion criteria：

①Individuals with acute illnesses, severe chronic diseases, or in the acute phase of chronic diseases, as well as those with fever. ②Individuals with a history of allergy to influenza vaccine or its components. ③Other conditions deemed unsuitable for influenza vaccination by physicians.

本研究将根据不同的经济收入水平和地域特征，确定3个研究城市，采用目的抽样在各城市内抽取3家社区卫生服务中心，共计9家。从社区卫生服务中心获取辖区内60岁以上老年人的电话号码，采用简单随机抽样方法进行样本选择。利用R 4.3.2软件中的“sample( )”函数进行随机抽取，将电话号码储存在向量“phone_numbers <- c( )”中，并设定抽样的样本量a，采用“random_number <- sample(phone_numbers, a)”和“print(random_number)”得到简单随机抽样的结果。

Randomization Procedure (please state who generates the random number sequence and by what method)：

According to different economic income levels and regional characteristics, this study will determine 3 study cities, and select 3 community health service centers in each city by purpose sampling, totaling 9. The telephone numbers of the elderly over 60 years old were obtained from the community health service center, and the sample was selected by simple random sampling method. The function "sample()" in R 4.3.2 software is used for random sampling, phone numbers are stored in the vector "phone_numbers < -c ()", and sample size a is set for sampling. "random_number < -sample (phone_numbers, a)" and "print(random_number)" are used to get the result of simple random sampling.

盲法：

由于干预对研究参与者基本上不存在生理/心理的风险与不适，故在招募通知及知情同意书中不透露任何干预措施的具体内容（已获得伦理委员会批准），保证研究参与者不受沾染或干扰。执行干预的调查员仅获得与其对应的干预文本和调查问卷，不同组别的调查员分开培训，确保他们互不知悉其他组的干预内容。

Blinding：

The interventions we implemented in our trial are a class of behavioral interventions, which are impossible to conducted in a blind trial. Nevertheless, we will try our best to apply several blinding approaches during the study procedure to reduce potential biases according to a previously reported protocol. Firstly, the process to generate the sequence of random digits for selecting and allocating participants will be kept confidential from all on-site research assistants who recruit and allocate participants. Secondly, all interventions will be concealed from study participants?until they are in the trial, as our interventions will not pose a risk to human health. Thirdly, the on-site research assistants in charge of interventions for each arm will be trained separately to ensure that they will not aware of the intervention content of the other arms. Finally, all interventions will be blinded to the public physicians who provide influenza vaccine service and delivery.
