研究目的：

通过临床随机对照试验，探讨经鼻肠管途径进行肠道菌群调节对于颅脑创伤患者感染防治及免疫功能调控的影响。  

Objectives of Study：

Investigating the impact of regulating intestinal flora through the nasointestinal tube route on infection prevention and immune function regulation in traumatic brain injury patients through clinical randomized controlled trials.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.伤后12小时内入住NICU的成年患者（>18岁）； （2）GCS评分3-8分； （3）临床和CT证实为存在颅脑损伤，且均为闭合性损伤； （4）急性胃肠功能损伤（AGI）分级小于III级； （5）预计生存时间>7天； （6）签署知情同意书。

Inclusion criteria

1.- Adult patients (>18 years old) admitted to the NICU within 12 hours after injury;
2.- Glasgow Coma Scale (GCS) score of 3-8 points;
3.- Clinical and CT confirmation of closed head injury;
4.- Acute gastrointestinal injury (AGI) grade less than III;
5.- Expected survival time exceeding 7 days;
6.- Signed informed consent.

排除标准：

1.既往消化道病史，严重器质性消化道疾病或其他原因造成伤后24-48h内无法启动肠内营养；
2.（8）其他原因导致研究提前结束；
3.（9）研究参与者或委托人收回知情同意。
4.（2）合并严重的肺部创伤或腹部外伤；
5.（3）生命体征不稳定（去甲肾上腺素>0.2ug/kg/min，多巴胺>2ug/kg/min），如积极复苏难以纠正的休克；
6.（4）肿瘤、血液病、糖尿病或其他慢性免疫功能异常基础疾病；
7.（5）氨苄青霉素、头孢唑啉、头孢呋辛、四环素、氯霉素、呋喃唑酮（痢特灵）、复方新诺明和氟哌酸药物；
8.（6）妊娠期妇女；
9.（7）7天内死亡；

Exclusion criteria：

1. History of severe organic gastrointestinal diseases or other reasons that prevent enteral nutrition within 24-48 hours after injury;
2.(2) Severe lung trauma or abdominal trauma;
3.(3) Unstable vital signs (norepinephrine >0.2ug/kg/min, dopamine >2ug/kg/min), such as shock that is difficult to correct with aggressive resuscitation;
4.(4) Underlying diseases such as tumors, blood diseases, diabetes, or other chronic immunodeficiency disorders;
5.(5) Allergies to penicillin, cephalosporins, cephamycins, tetracyclines, chloramphenicol, nitrofurantoin, metronidazole, and fluoroquinolones;
6.(6) Pregnant women;
7.(7) Death within 7 days;
8.(8) Premature termination of the study due to other reasons;
9.(9) Withdrawal of informed consent by participants or their representatives.
