研究目的: |
经颅直流电刺激(tDCS) 是一种无创性的,利用恒定、低强度直流电调节大脑皮层神经元活动的神经电刺激技术,通过在头皮表面放置电极实现对特定脑区施加不同极性(正或负)的微弱电流刺激,tDCS 可促使作用区域的神经元静息膜电位发生改变,促使神经元的动作电位发生概率改变,进而改变相应脑区神经元的兴奋性[1-2]。 焦虑和抑郁障碍共病患者的发病与其脑电活动异常有关[3-4],具有病情严重、病程长、症状反复、预后差等特点,严重影响患者及家属的正常生活与工作;治疗处理工作较为棘手,成为当今全球疾病负担的主要原因及精神病学领域最关注的焦点问题之一,得到了医务工作者的广泛关注[5-6]。目前,药物是治疗焦虑和抑郁障碍共病的主要方法,但依从性低、不良反应发生率高,tDCS 已经成为精神病学、认知神经科学、神经康复医学的研究热点,且tDCS已广泛应用于精神科临床,特别是针对抑郁障碍治疗。目前普遍认为tDCS 可能的作用可能与能改善大脑皮质兴奋性、改变局部脑血流量、增加神经突触可塑性、调节局部皮质间的网络连接、影响脑内神经递质等有关。近年来,随着人们生活及工作压力的增加,导致失眠发生率不断升高,并呈现出年轻化的趋势[7]。失眠会导致患者出现一系列睡眠生理紊乱情况,进而引起焦虑、抑郁等负面情绪,同时还会使机体免疫力下降,出现消化道疾病及血糖、血压升高等多种病症。[8-9]失眠会与焦虑、抑郁情绪相互影响,失眠会产生焦虑、抑郁情绪,而焦虑、抑郁情绪 会导致失眠程度加重[10]。失眠患者主诉存在头晕、头痛、记忆力减退等情况,对其生活及工作产生了较大影响,尤其是部分存在焦虑、抑郁的失眠患者还会出现注意力分散、学习记忆能力下降、认知执行功能下降等情况,出现认知功能障碍,进一步降低生活质量;我们此次研究是通过自有脑电算法于F3点位和F4点位进行经颅直流电刺激治疗结合特定音乐疗法来治疗焦虑抑郁失眠患者,舒压助眠。将认知评估与神经调控技术相结合,形成检测和干预闭环系统。 |

Objectives of Study: |
Transcranial direct current stimulation (tDCS) is a non-invasive, neuroelectric stimulation technique that utilizes constant, low-intensity direct current to modulate neuronal activity in the cerebral cortex. The onset of anxiety and depression disorders in patients with co-morbidities is related to the abnormalities of their electroencephalographic activity [3-4], which is characterized by seriousness, long duration of the disease, recurrent symptoms, and poor prognosis, and seriously affects the normal life and work of patients and their families. Our current study is to treat patients with anxiety, depression, and insomnia by using our own EEG algorithm for transcranial direct current stimulation therapy at points F3 and F4 combined with specific music therapy to relieve stress and help sleep. Cognitive assessment and neuromodulation techniques are combined to form a closed-loop system of detection and intervention. |

1: 年龄18-65岁之间 2: 匹兹堡睡眠质量指数(Pitts- burgh Sleep Quality Index, PSQI)评分≥7 分 3: 入组前两周均未使用过抗精神病、镇静安眠等可能对症状产生影响的药物 4: 无严重躯体疾病、脑血管意外或脑器质性疾病 5: 依从性好,可配合本研究 |

1: between 18 and 65 years of age 2: Pitts- burgh Sleep Quality Index (PSQI) score ≥7 3: no antipsychotic, sedative or hypnotic medications were used in the two weeks prior to enrollment that might have an effect on symptoms 4: no serious physical illness, cerebrovascular accident or organic brain disease 5: compliance was good enough to cooperate with this study |
