通过多中心随机对照临床试验,评估基于食管内镜视频帧序列质量分类算法的人工智能系统在实时辅助评估和提高食管内镜检查质量中的作用。  

Objectives of Study:

Evaluate the role of the artificial intelligence system based on esophageal endoscopic video frame sequence quality classification algorithm in real-time assisted evaluation and improvement of esophageal endoscopic examination quality through a multicenter randomized controlled clinical trial.

药物成份或治疗方案详述:

 

Description for medicine or protocol of treatment in detail:

 

纳入标准:

① 年龄大于等于18周岁,小于等于80周岁; ② 适合接受上消化道内镜检查; ③ 患者及家属知情同意; ④ 无已知的食管解剖结构异常,包括食管憩室、食管气管瘘、食管疾病手术史、食管癌放疗史等。

Inclusion criteria

① Age 18 to 80 years old; ② Suitable for upper gastrointestinal endoscopy examination; ③ Informed consent of patients and their families; ④ There are no known anatomical abnormalities in the esophagus, including esophageal diverticulum, esophagotracheal fistula, history of esophageal disease surgery, and history of esophageal cancer radiotherapy.

排除标准:

① 妊娠期妇女; ② 不能暂停抗凝/抗血小板治疗; ③ 存在凝血障碍(PLT < 50 × 103/μL, INR > 1.5)。

Exclusion criteria:

① Pregnant women; ② Cannot suspend anticoagulant/antiplatelet therapy; ③ There is coagulation disorder (PLT<50 × 103/μ L, INR>1.5).

胃镜检查退镜时开启“真”AI质控系统,当实时检测到的高质量内镜图像占比过低时,该AI质控系统将发出警报信号示意内镜医生注意精细操作。

在胃镜检查前由中间人(与本课题组无关人员)将内含随机数字标签的密封的不透明信封分配给受试者,标签上为奇数(从1到148)的受试者纳入试验组,偶数的纳入对照组,随后由中间人开启“真”或“伪”AI质控系统。

Randomization Procedure (please state who generates the random number sequence and by what method):

Before the gastroscopy examination, a sealed opaque envelope containing a random number label was assigned to the subjects by an intermediary (unrelated to our research group). Participants with odd numbers (from 1 to 148) on the label were included in the experimental group, while those with even numbers were included in the control group. Subsequently, the intermediary activated the "true" or "false" AI quality control system.

数据采集和管理(说明:数据采集和管理由两部分组成,一为病例记录表(Case Record Form, CRF),二为电子采集和管理系统(Electronic Data Capture, EDC),如ResMan即为一种基于互联网的EDC:

使用EDC进行数据采集和管理。由通过统一培训的研究者根据原始观察记录,将数据及时、正确、完路、清晰地记录在病例报告表。由临床监查员审查后,移交给本中心数据录入员,经过培训的数据录入员负责数据录入与管理工作。数据录入员通过登陆复旦大学附属中山医院临床科研数据管理平台 (EDC系统),在线进行数据录入,为保证数据的准确性,各分中心由数据录入员独立单份录入。初步提交到平台的数据由平台质控员进行数据质量监查,通过反馈《数据质量疑问表》或在线质疑系统,对系统自带逻辑核查、一致性核查校对之外的,具有医学内涵的数据质量进行监查、反馈,确保数据的质量。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

Use EDC for data acquisition and management. According to the original observation records, the data should be recorded in the case report form in a timely, correct, complete and clear manner by the researchers who have passed the unified training. After being reviewed by the clinical monitor, it is transferred to the data entry clerk of the center, and the trained data entry clerk is responsible for data entry and management. The data entry clerk logs in the Clinical Research Data Management platform (EDC system) of Zhongshan Hospital Affiliated to Fudan University to enter the data online. In order to ensure the accuracy of the data, the data entry clerk enters the data independently in each branch center. The data initially submitted to the platform will be monitored by the platform quality control officer, and the data quality with medical connotation outside of the system's own logical verification and consistency verification will be monitored and fed back through the data quality Query Form or the online query system to ensure the quality of the data.
