研究目的：

构建智慧慢病药学服务平台和慢性肾脏病（CKD）患者的全程药物治疗管理体系，形成以患者为中心的药学服务闭环，实现院内院外/线上线下同质化药学服务，并评估该体系用于CKD慢病管理的效果。  

Objectives of Study：

To construct an intelligent chronic disease pharmaceutical service platform and medication therapy management system for patients with chronic kidney disease (CKD), form a patient-centered closed loop of pharmaceutical care, realize homogeneous pharmaceutical care within and outside the hospital/online and offline, and evaluate the effect of the system in chronic disease management of CKD.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.符合慢性肾脏病诊断的患者。
2.肾脏 ECT 检测 eGFR＜60mL·min-1·1.73m-2 或 CKD-EPI 肾功能估算公式计算 eGFR＜60mL·min-1·1.73m-2。

Inclusion criteria

1.Meet the diagnostic criteria of chronic kidney disease; 2.EGFR < 60mL·min-1·1.73m-2 was detected by renal ECT or eGFR < 60mL·min-1·1.73m-2 was calculated by CKD-EPI renal function estimation formula.

排除标准：

1.已行肾脏替代治疗（包括肾移植、血液透析和腹膜透析）；
2.年龄＞75 岁或年龄＜18 岁；
3.语言交流困难；
4.有智力障碍、精神病史、抑郁症、焦虑症或老年痴呆；

Exclusion criteria：

1.Patients who have received renal replacement therapy (including kidney transplantation, hemodialysis, and peritoneal dialysis);
2.The patient's age was > 75 years old or < 18 years;
3.Patients with difficulty in language communication;
4.Patients with mental retardation, a history of mental illness, depression, anxiety, or dementia.
