研究目的：

微创心脏手术后切口疼痛会增加患者围术期风险并严重影响预后。本研究拟观察右美托咪定作为佐剂用于前锯肌神经阻滞的术中、术后3个月镇痛效果，观察其对心脏病患者围术期心肌保护作用，为此类患者临床合理选用镇痛方案提供理论依据。  

Objectives of Study：

Incision pain after minimally invasive cardiac surgery will increase the perioperative risk of patients and seriously affect the prognosis. The purpose of this study was to observe the analgesic effect of dexmetomidine as an adjuvant on anterior serratus nerve block during and 3 months after operation, and to observe its perioperative myocardial protection in patients with heart disease, so as to provide a theoretical basis for the rational selection of analgesia regimen for such patients.

纳入标准：

1、小切口微创心脏手术患者 2、年龄18-75岁 3、ASA 分级 II-IV 级

Inclusion criteria

1. Patients undergoing minimally invasive cardiac surgery 2. Patients aged 18 to 74 years 3. ASA class II or Ⅳ

排除标准：

1、恶性肿瘤患者 2、合并严重感染患者 3、合并严重肝、肾、肺部疾病及凝血功能失调患者 4、二次心脏手术 5、慢性疼痛综合征 6、慢性抗炎药物及阿片类药物应用 7、精神疾病不能配合者 8、出血量大于全身血容量50%者 9、孕产妇 10、实验药物过敏者 11、周围神经病变 12、免疫抑制治疗或激素治疗 13、术中改行正中开胸手术者

Exclusion criteria：

1. Malignant tumor 2. Serious infection 3. Preexisting major liver, kidney or lung disease, pulmonary hypertension, or abnormal blood clotting function 4. Cardiac reoperation 5. Chronic pain syndrome 6. Application of chronic anti-inflammatory drugs and opioid drugs 7. Individuals with mental illness who cannot cooperate 8. Blood loss is greater than 50% blood volume 9. Maternal 10. Experimental drug allergy 11. Peripheral neuropathy 12. Immunosuppressive therapy or hormone therapy 13. Intraoperative conversion to median thoracotomy
