研究目的：

通过前瞻性临床研究，纳入上海市中医医院肿瘤科门诊及病房III期结直肠癌术后患者，评价健脾通络方治疗III期结直肠癌术后患者临床疗效及安全性。此外我们基于16s rRNA测序及代谢组学测序收集患者治疗前后粪便样本，从肠道菌群及代谢产物的角度出发，对健脾通络方的临床疗效机制进行探索，为中西医结合治疗结直肠癌提供临床依据。  

Objectives of Study：

Through prospective clinical research, we included patients with stage III colorectal cancer who underwent surgery in the outpatient department and ward of Shanghai Traditional Chinese Medicine Hospital, and evaluated the clinical efficacy and safety of Jianpi Tongluo Formula in treating postoperative patients with stage III colorectal cancer. In addition, we collected fecal samples from patients before and after treatment based on 16s rRNA sequencing and metabolomics sequencing. From the gut microbiota and perspective of metabolites , we explored the clinical efficacy mechanism of Jianpi Tongluo Formula, providing clinical evidence for the combination of traditional Chinese and Western medicine in the treatment of colorectal cancer.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1） 符合上述诊断标准诊断为结直肠癌Ⅲ期术后； （5） 年龄≥18岁，性别不限； （6） 预计生存期≥6个月的患者，体力状况评分标准（ECOG）≤2； （7） 同意接受随访； （8） 随访及临床相关病历资料完整； （8） 同意采集用药前后粪便样本进行微生物组及代谢组检测； （9） 自愿签署知情同意书者。

Inclusion criteria

(1) Diagnosed as stage III postoperative colorectal cancer according to the above diagnostic criteria; (2) Age ≥ 18 years old, regardless of gender; (3) For patients with an expected survival time of ≥ 6 months, the physical condition scoring standard (ECOG) is ≤ 2; (4) Agree to receive follow-up; (5) Complete follow-up and clinical related medical records; (6) Agree to collect fecal samples before and after medication for microbiological and metabolomic testing; (7) Those who voluntarily sign an informed consent form.

排除标准：

（1） 合并其他原发恶性肿瘤； （2） 合并严重心、脑、肝、肾疾病、电解质异常或精神病者； （3） 对本研究所用药物或其成分过敏者； （4） 妊娠期、哺乳期或计划妊娠患者； （5） 存在严重或难以控制的感染； （6） 近2周内服用其他中药者或参加其它临床试验者。

Exclusion criteria：

(1) Merge with other primary malignant tumors; (2) Individuals with severe heart, brain, liver, kidney diseases, electrolyte abnormalities, or mental illness; (3) Individuals who are allergic to the drugs or their components used in this study; (4) Pregnant, lactating, or planned pregnancy patients; (5) Serious or uncontrollable infections exist; (6) Individuals who have taken other traditional Chinese medicine or participated in other clinical trials within the past 2 weeks.

健脾通络方联合III期结直肠癌术后辅助化疗方案，健脾通络方：生黄芪30g、苦参15g、党参15g、炒白术15g、白茯苓15g、炒薏苡仁15g、白花蛇舌草30g、天龙3g、蜈蚣3g、甘草6g，所有药物均为我院中药房提供的中药饮片，每日一剂，水煎附，每剂约400ml，早晚两次饭后温服。给药疗程6个月。

Intervention：

Jianpi Tongluo Formula combined with adjuvant chemotherapy for postoperative stage III colorectal cancer. Jianpi Tongluo Formula includes 30g of raw Astragalus membranaceus, 15g of Sophora flavescens, 15g of Codonopsis pilosula, 15g of stir fried Atractylodes macrocephala, 15g of Poria cocos, 15g of stir fried Coix seed, 30g of Hedyotis diffusa, 3g of Tianlong, 3g of centipede, and 6g of licorice. All drugs are traditional Chinese medicine slices provided by our hospital's traditional Chinese medicine department, one dose per day, decoction with water, each dose is about 400ml, and taken warm after meals twice in the morning and evening. The medication course is 6 months.
