确定围术期呼吸六字诀训练对心脏外科手术患者术后肺部并发症的疗效和安全性  

Objectives of Study：

To determine the efficacy and safety of perioperative six character formula training on postoperative pulmonary complications in elderly patients under general anesthesia

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）年龄≥60 周岁，性别不限；
（2）拟行择期气管插管全身麻醉下骨科手术、胃肠手术、肝胆胰腺手术、胸科手术；
（3）美国麻醉医师协会（ASA） I～III级；
（4）手术时间≥2小时；
（5）患者自愿参加并签署知情同意书；
（6）患者可配合获得结果测量的完整信息。

Inclusion criteria

（1）Patients aged ≥60 years （2）Patients undergoing elective orthopedic surgery, gastrointestinal surgery, hepatobiliary pancreatic surgery, and thoracic surgery under general anesthesia with elective tracheal intubation; （3）ASA I-III （4）Operation time ≥2 hours; (5) Patients voluntarily participated and signed informed consent form; （6）Patients cooperate with that training and obtain complete result information

排除标准：

（1）最近3个月内有急性肺损伤或急性呼吸窘迫综合征的术前病史；
（2）术前60天内有气管插管或机械通气史；
（3）慢性阻塞性肺病 (COPD)、阻塞性睡眠呼吸暂停综合征、哮喘、限制性肺病、术前严重胸部感染如肺结核、胸部畸形如鸡胸、漏斗胸、胸腰椎侧凸、膈疝等病史
（4）术前长期使用激素治疗（≥3个月）
（5）术后7天内可能再次手术者；
（6）并存危重疾病，包括严重的肝功能障碍(如肝功能衰竭：血清总胆红素升高，≥171umol/L或每日升高≥17.1umol/L；凝血酶原活动度≤40％)、慢性肾功能衰竭(肾小球滤过率< 30 mL/min)；
（7）体重指数（BMI）＞30kg/m2；
（8）认知或神经缺陷患者；
（9）急诊手术；

Exclusion criteria：

(1)Preoperative history of acute lung injury or acute respiratory distress syndrome in last 3 months; (2)History of intubation or mechanical ventilation within 60 days prior to surgery; (3)Chronic obstructive pulmonary disease (COPD),obstructive sleep apnea ,asthma, restrictive lung disease, preoperative major chest infection, e.g. pulmonary tuberculosis, chest deformities such as pectus carinatum, pectus excavatum, thoracolumbar scoliosis, diaphragmatic hernias diagnosed on history; (4)Patients who have long-term use of hormone therapy before surgery (≥3 months); (5)Patients who may have reoperation within 7 days after surgery; (6)Critical illness, including severe hepatic dysfunction (eg, hepatic failure), chronic renal failure (glomerular filtration rate <30 mL/min); (7)BMI＞30kg/m2 (8) Patients with cognitive or neurological defcits; (9)Emergency operation;
