研究目的：

本项目通过在成都市玉林社区卫生服务中心开展居家药学服务工作，通过对其进行科学论证评价与研究，探索开展居家药学服务工作的可行性，建立居家药学社区服务工作模式，为我省制定居家药学服务工作服务标准，开展居家药学服务工作提供理论及实践依据。  

Objectives of Study：

This project explores the feasibility of home pharmacy service by carrying out home pharmacy service work in Yulin community health service center, and through scientific demonstration, evaluation and research, establishes a home pharmacy community service work model, and provides theoretical and practical basis for formulating home pharmacy service service standards and carrying out home pharmacy service work in our province.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

(1) 成都市玉林社区卫生服务中心家庭医生签约的规范化管理的诊断为糖尿病的患者，同时服用 3 种及以上药物，或合并其他多病种联合用药； (2) 年龄35-75 岁，病程＞1 年，性别不限； (3) 签署知情同意书，并要求患者在实行药物治疗管理期间不得擅自更改药物或停药，接受定期随访者。 (4) 能够熟练操作智能手机的患者。

Inclusion criteria

(1) Patients with the diagnosis of essential diabetes contracted by a community family doctor in Chengdu who are taking 3 or more drugs at the same time, or combining drugs with other diseases; (2) Age 35-75 years, disease duration > 1 year, gender is not limited; (3) Patients who sign the informed consent form and require the patient not to change the drug or stop the drug without authorization during the implementation of drug treatment management, and accept regular follow-up. (4) Patients who are proficient in operating smartphones.

排除标准：

(1) 不符合研究对象纳入标准者，比如继发性高血压、糖尿病患者; 急性心肌梗死和脑卒中急性期( ＜ 3 个月) 患者; (2) 不易配合( 智力、听力、肢体活动明显障碍) 患者; (3) 合并严重疾病，预期寿命不足 1 年患者； (4) 因精神障碍、严重活动障碍等疾病无法参与本项目研究者； (5) 正在参与本项目类似研究项目者； (6) 不依从本项目所应用干预措施者。

Exclusion criteria：

(1) Patients who do not meet the inclusion criteria of the study subjects, such as patients with secondary hypertension and diabetes; Patients with acute myocardial infarction and acute phase of stroke (< 3 months); 2) Patients who are not easy to cooperate (obvious impairments of intellectual, hearing, and limb movements); 3) Patients with severe illness and life expectancy of less than 1 year; (4) Patients who are unable to participate in this project due to mental disorders, severe activity disorders and other diseases; (5) Patients who are participating in similar research projects in this project; (6) Patients who do not comply with the interventions applied in this project.

干预包括临床医师日常诊疗工作及用药。干预组在社区家庭医师团队基础上加入1名家庭药师，家庭药师的工作内容如下： (1) 建立药物治疗档案1份，包括：采集患者用药及相关信息（包括患者基本信息，既往用药史、相关检查、目前使用药物情况），评估居家患者药物治疗需求；用药清单，记录药品不良反应，药物相互作用筛查，用药方案调整建议，填写访视表。 (2) 用药随访(3次，1个月，3个月，6个月)：包括：药品不良事件筛查；药物相互作用筛查；科普宣教；用药教育 (3) 服药依从性的调查2次（首次，6个月）； (4) 用药咨询门诊3次； (5) 清理家庭药箱1次：药师可指导有需要的居家患者清理家庭药箱，关注家中药品的有效期、性状和储存条件等，对居家患者进行药品整理、分类存放、过期或变质药品清理提供服务指导建议；

Intervention：

The intervention included the daily work of clinicians and medication. In the intervention group, a family pharmacist was added to the community family physician team, and the work of the family pharmacist was as follows: (1) Establish 1 drug treatment file, including: collecting patients' medication and related information (including basic information of patients, previous drug history, relevant examinations, and current drug use), assessing the drug treatment needs of patients at home, drug list, recording adverse drug reactions, drug interaction screening, medication plan adjustment suggestions, and filling in the visit form. (2) Medication follow-up (3 times, 1 month, 3 months, 6 months): including: drug adverse event screening, drug interaction screening, science education, drug education (3) 2 surveys on medication compliance (first time, 6 months); (4) 3 outpatient consultations; (5) Clean the home medicine cabinet once: the pharmacist can guide the home patients in need to clean the home medicine box, pay attention to the expiration date, characteristics and storage conditions of the drugs at home, and provide guidance and suggestions for the home patients to sort out the drugs, classify and store them, and clean up expired or deteriorated drugs;
