研究目的：

使用一种新型的区域神经阻滞方法，对乳腺手术患者通过神经支、末梢阻滞替代全身麻醉，使老年、心肺功能较差等不能耐受全身麻醉的患者能够通过这种麻醉方式完成手术。  

Objectives of Study：

A novel nerve block method was used to replace general anesthesia with nerve branch and terminal block for breast surgery patients, so that elderly patients with poor cardiopulmonary function who could not tolerate general anesthesia could complete the operation through this anesthesia method.

药物成份或治疗方案详述：

对诊断明确，需行择期乳腺手术但由于高龄、心肺功能差等原因不能耐受全身麻醉的患者，通过采取区域神经阻滞的方法，对患者术区部位行神经支、末梢阻滞，观察术中患者血压、心率的变化及阿片类药物使用量，记录术后并发症如术后恶心呕吐、术后疼痛等，同时与实施常规全身麻醉方法的患者进行对比，评估区域神经阻滞替代全身麻醉在乳腺手术中的可能性。 

Description for medicine or protocol of treatment in detail：

For patients who need elective breast surgery but cannot tolerate general anesthesia due to old age, poor cardiopulmonary function and other reasons, regional nerve block is adopted to perform nerve branch and terminal block at the operative area of the patients, observe the changes of intraoperative blood pressure,heart rate and intraoperative doses of opioids used, and record postoperative complications such as postoperative nausea and vomiting and postoperative pain. At the same time, the possibility of replacing general anesthesia with regional nerve block in breast surgery was evaluated in comparison with patients undergoing conventional general anesthesia. 

纳入标准：

（1）区域神经阻滞组 ①经专科会诊因心肺功能差等原因不能耐受或者不建议在全身麻醉下完成手术，患者同意使用区域神经阻滞的麻醉方法完成手术； ②对全身麻醉恐惧，不愿行全身麻醉的乳腺疾病患者； ③无精神疾病病史； ④无局麻药过敏史。 （2）全身麻醉组 ①诊断明确，需行择期乳腺手术的患者； ②美国麻醉医师协会（ASA）分级Ⅰ~Ⅲ级； ③根据年龄（±5岁），体重（±8kg）以及相同的手术方式，能与区域神经阻滞组进行匹配成功的潜在患者； ④无精神疾病病史。

Inclusion criteria

(1)Regional nerve block group ①After specialist consultation, the patient cannot tolerate or is not recommended to complete the operation under general anesthesia due to poor cardiopulmonary function and other reasons, and the patient agrees to use the anesthesia method of regional nerve block to complete the operation; ②Fear of general anesthesia, do not want to do general anesthesia patients with breast diseases; ③No history of mental illness; ④No allergic history of local anesthetics. (2) General anesthesia group ①Patients with a definite diagnosis who need elective breast surgery; ② American Society of Anesthesiologists (ASA) Level I ~ Ⅲ; ③Potential patients who can be successfully matched with the regional nerve block group based on age (±5 years), weight (±8 kg), and the same surgical method; ④No history of mental illness.

排除标准：

（1）区域神经阻滞组 ①穿刺部位皮肤和深层组织有感染病灶； ②出血倾向或正在进行抗凝治疗者； ③妊娠期； ④不能正确交流沟通、不能配合的患者； （2）全身麻醉组 ①心肺功能不全，肝肾功能异常的患者； ②对麻醉药物过敏； ③妊娠期患者； ④拒绝全身麻醉的患者。

Exclusion criteria：

(1)Regional nerve block group ①There are foci of infection in the skin and deep tissues at the puncture site; ② Those who have bleeding tendencies or are on anticoagulation therapy; ③ Patients in pregnancy; ④ Patients who are unable to communicate properly or are unable to cooperate; (2) General anesthesia group ①Patients with poor cardiopulmonary function or abnormal liver and kidney function; ②Allergic to narcotic drugs; ③Patients in pregnancy; ④Patients who refuse general anesthesia.
