研究目的：

体外膜肺氧合（ECMO）是体外生命支持技术，用于挽救可逆性心肺功能衰竭的患者，传统上受到抗凝治疗要求的限制。尽管对ECMO设备例如离心泵和肝素涂层的回路进行了技术改进，但出血仍是ECMO患者最常见的并发症。尤其是对于高危出血风险和存在活动性出血的患者。最近国外的一些个案病例报道，采取了短期无肝素方法应对这类患者。因此本研究我们比较无肝素抗凝与常规肝素抗凝的ECMO患者之间在存活率、出血情况、血栓形成、输血需求以及更换氧合器的次数，探索无肝素ECMO治疗的有效性及安全性。  

Objectives of Study：

Extracorporeal membrane oxygenation (ECMO) is an extracorporeal life support technology used to rescue patients with reversible heart and lung failure, traditionally limited by the requirements of anticoagulation therapy. Despite technical improvements to ECMO equipment such as pumps and heparin-coated circuits, bleeding is still the most common complication in patients with ECMO. Especially for patients at high risk of bleeding and active bleeding. Recently, some foreign cases have reported that a short-term heparin-free approach has been adopted to deal with such patients. Therefore, in this study, we compare the survival rate, bleeding, thrombosis,transfusion requireent, and the number of oxygenator changes between ECMO patients without heparin and conventional heparin, and explore the effectiveness and safety of heparin-free ECMO .

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 年龄≤65岁；
2. 预计为可逆原因造成的心肺衰竭；
3. ELSO推荐的适应症；
4. 知情同意。

Inclusion criteria

1. Age ≤ 65 years;
2. With cardiopulmonary failure due to reversible causes;
3. With indications recommended by ELSO;
4. Patients with informed consent.

排除标准：

1. 年龄＜18岁；
2. 心脏骤停后出现不可逆的脑损伤；
3. 濒死状态；
4. 恶性肿瘤终末期；
5. 存在不可逆的严重中枢神经系统疾病如脑疝形成出现平坦的脑电波；
6. 血管异常无法完成ECMO的穿刺置管操作。

Exclusion criteria：

1. Age <18 years;
2. Patients with irreversible brain damage after cardiac arrest;
3. On the verge of death;
4. Patients with terminal stage of malignant tumor;
5. With irreversible serious central nervous system diseases such as cerebral hernia formation and flat brain waves;
6. Patients with abnormal blood vessels unable to complete ECMO catheterization.
