研究目的：

（1）构建基于自我调节常识模型的冠心病患者心脏运动康复依从性干预方案； （2）将干预方案应用于临床患者，并对方案应用效果进行评价，为临床医护人员提高患者运动康复依从性提供新思路。  

Objectives of Study：

(1) To construct an intervention plan for compliance of coronary heart disease patients with cardiac exercise rehabilitation based on self-regulation common sense model; (2) Apply the intervention scheme to clinical patients, and evaluate the application effect of the scheme, so as to provide new ideas for clinical medical staff to improve the compliance of patients with exercise rehabilitation.

药物成份或治疗方案详述：

（1）初步构建干预方案 采用文献研究方法，查阅冠心病患者心脏康复、运动康复以及护理干预等相关循证指南和证据总结，在参照国际有关冠心病患者心脏康复护理指南的基础上，初步构建冠心病患者的运动康复方案。 （2）最终形成运动康复方案 通过小组讨论、德尔菲专家函询和预实验等方法对方案初稿进行修订，并最终形成科学、规范的基于自我调节常识模型的冠心病患者运动康复依从性干预方案。 （3）初步应用干预方案 采用类试验的研究方法，以长沙市某三甲医院心内科的冠心病患者为研究对象实施为期12周的干预，并通过运动依从性问卷、简易疾病感知问卷、冠心病运动恐惧量表、情绪调节自我效能以及临床健康指标等方面评价该方案对冠心病患者的应用效果。 

Description for medicine or protocol of treatment in detail：

(1) Preliminary construction of intervention plan By using the literature research method, the author consulted the relevant evidence-based guidelines and evidence summary of cardiac rehabilitation, exercise rehabilitation and nursing intervention for patients with coronary heart disease, and on the basis of referring to the international cardiac rehabilitation nursing guidelines for patients with coronary heart disease, the exercise rehabilitation program for patients with coronary heart disease was initially constructed. (2) Finally form a sports rehabilitation program Through group discussion, Delphi expert letter consultation and pre-experiment, the first draft of the program was revised, and finally a scientific and standardized intervention plan for exercise rehabilitation compliance of patients with coronary heart disease based on the common sense model of self-regulation was formed. (3) Initial application of intervention programs By using the trial-like research method, patients with coronary heart disease (CHD) in the Department of Cardiology of a Grade-three hospital in Changsha were selected as the research objects for a period of 12 weeks, and the effect of the program on CHD patients was evaluated through exercise compliance questionnaire, simple disease perception questionnaire, exercise fear scale of CHD, emotional regulation self-efficacy and clinical health indicators.

纳入标准：①年龄≥18岁；②符合 WHO冠心病诊断标准的冠心病患者；③意识清楚，有一定的语言表达能力；④能够简单使用微信或能在指导下使用微信，并愿意接受微信或电话随访者；⑤NYHA分级心功能Ⅰ级~Ⅲ级；⑥患者知情同意并自愿参与本研究；

Inclusion criteria

Inclusion criteria: ① age ≥18 years old; (2) Patients with coronary heart disease WHO meet the diagnostic criteria for coronary heart disease; ③ Clear consciousness, have a certain language expression ability; Be able to use wechat easily or under guidance, and be willing to accept wechat or telephone interviewers; ⑤NYHA grade I ~ III cardiac function; (6) Informed consent of patients and voluntary participation in this study;

排除标准：

排除标准：①听力、视力严重障碍者；②存在精神异常、认知障碍者；③合并其他严重疾病的患者；④运动性功能障碍或肢体残疾者；⑤存在运动康复禁忌症：如不稳定性心绞痛、急性心肌梗死者、未控制的心律失常、血压＞220mmHg、恶性肿瘤等患者。

Exclusion criteria：

Exclusion criteria: ① severe hearing and vision impairment; ② There are mental abnormalities, cognitive disorders; ③ Patients with other serious diseases; ④ Motor dysfunction or physical disability; ⑤ There are contraindications for exercise rehabilitation: such as unstable angina pectoris, acute myocardial infarction, uncontrolled arrhythmia, blood pressure > 220mmHg, and malignant tumors.

采用Excel生成随机数字表，研究人员根据随机数字表按顺序将患者分为试验组与对照组，给研究对象发放不透明密封信封，提前规定抽到信封内数字为奇数，分配到试验组，数字为偶数，分配到对照组。参与者在分组随机化之前被确定和招募，以避免产生招募偏倚。

Excel was used to generate a random number table, and the researchers divided the patients into test group and control group in order according to the random number table. The study subjects were given opaque sealed envelopes, and the numbers drawn in the envelopes were odd and assigned to the test group, and the numbers were even and assigned to the control group. Participants were identified and recruited prior to group randomization to avoid recruitment bias.
