研究目的：

本项临床试验的目的是为了解肠内生态免疫营养对胃肠道肿瘤患者化疗期间营养状况、免疫功能和化疗耐受的影响。  

Objectives of Study：

The goal of this clinical trial is to learn about impact of enteral ecoimmunonutrition on immunological response, nutritional status and tolerance to treatment in gastrointestinal malignancy patients receiving chemotherapy.

药物成份或治疗方案详述：

通过随机数字表法将患者随机分至对照组和生态免疫营养组，每组各14例。对照组给予患者基础性的健康宣教，嘱患者保持健康饮食，告知注意事项，正常饮食，不给予全优膳。生态免疫营养组患者在正常饮食基础上，患者从化疗第1天开始口服全优膳，每日1罐（237ml），共服用42天。全优膳（苏州东耀公司）：是一种专为肿瘤患者配制的全营养配方食品，富含乳清蛋白、酪蛋白、大豆蛋白等优质蛋白，菜籽油、红花籽油、亚麻籽油、鱼油等脂肪成分，单不饱和脂肪酸和多种不饱和脂肪酸（ω-3 PUFAs、ω-6 PUFAs），核苷酸、精氨酸等多种免疫营养素以及膳食纤维（包含低聚果糖和抗性糊精，二者均为益生元），每罐237mL，提供308 cal的热量（1.3 cal/mL高热能配方）。 

Description for medicine or protocol of treatment in detail：

Patients were randomly assigned to the control group and the EIN group using a random number table, with 14 cases in each group.In the control group, patients received essential health education (healthy diets and other precautions), and a regular diet (without the use of Ayunutri). In the EIN group, besides regular diet, patients were administered orally a jar (237 mL) of Ayunutri from the first day of chemotherapy for 42 days. Ayunutri (TOT BioPharm, Suzhou, Jiangsu Province, China) is a whole nutrition formula specially prepared for tumor patients. It is rich in high-quality proteins (e.g., lactalbumin, casein and soy bean protein), fat components (e.g., rapeseed oil, safflower seed oil, flax seed oil, and fish oil), monounsaturated fatty acids, polyunsaturated fatty acids (PUFAs) (e.g., ω-3 PUFAs and ω-6 PUFAs), immune nutrients (e.g., nucleotides and arginine), and dietary fibers (containing oligofructose and resistant dextrin, both of which are prebiotics). Each jar contains 237 mL of Ayunutri and can provide 308 calories (1.3 cal/mL). 

纳入标准：

①手术/内镜病理确诊为食管癌、胃癌、结直肠癌。②影像学评估为III-IV期（参照食管癌、胃癌、结直肠癌的AJCC第8版分期），伴有可评估病灶。③可经口进食。④预计生存期≥3个月。⑤需行全身化疗（双周或三周联合化疗方案）。⑥年龄在18-80岁（包含18岁和80岁）。

Inclusion criteria

① with a diagnosis of esophageal, gastric, or colorectal cancer confirmed by surgical/endoscopic pathology; ② the cancer was radiologically assessed as stage III or IV according to the American Joint Committee on Cancer (AJCC) staging manual (8th edition), along with the presence of assessable lesions; ③ the patient could be fed orally; ④ with an expected survival of ≥3 months; ⑤requiring systemic chemotherapy (biweekly or three-weekly combination chemotherapy regimens); and ⑥ aged 18 - 80 years (including 18 and 80 years).

排除标准：

①合并药物不能控制的肝肾功能不全及其他代谢性疾病等。②合并其他部位急慢性感染者。③使用激素和免疫抑制剂。④合并其他恶性肿瘤。⑤完全性肠梗阻。⑥不能经口进食。⑦对研究用药过敏及其他原因不能配合完成的研究者。

Exclusion criteria：

①with co-existing hepatic and renal insufficiency and other metabolic diseases that could not be controlled by medications; ② with acute or chronic infections at other sites; ③use of steroids or immunosuppressive agents; ④ with accompanying malignant tumors; ⑤ with complete intestinal obstruction; ⑥ could not be fed orally; and⑦ allergic to the study drug or unable to complete the study due to other reasons.
