研究目的：

评估2型糖尿病专家患者培养方案的效果及其可行性，为2型糖尿病专家患者培养提供参考。  

Objectives of Study：

To evaluate the effect and feasibility of type 2 diabetes expert patient training program, and to provide reference for type 2 diabetes expert training.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）符合《中国2型糖尿病防治指南》（2020 年版）诊断标准的 2 型糖尿病患者；病程≥5年；（2）年龄≥18岁；（3）初中及以上学历；（4）具有完全的认知和行为能力，能正常进行语言沟通者；（5）知情同意，自愿参与。

Inclusion criteria

(1) Patients with type 2 diabetes who meet the diagnostic criteria of the "Chinese Guidelines for the Prevention and Treatment of type 2 Diabetes" (2020 edition); Duration ≥5 years; (2) Age ≥18 years old; (3) Junior high school education or above; (4) Those who have complete cognitive and behavioral abilities and can normally communicate in language; (5) Informed consent and voluntary participation.

排除标准：

（1）合并严重的急、慢性糖尿病并发症导致身体活动受限者；（2）合并恶性肿瘤等其他严重疾病；（3）认知功能障碍或精神疾病史；（4）同时参加其他研究项目者。

Exclusion criteria：

(1) Patients with severe acute or chronic diabetes complications resulting in limited physical activity; (2) With other serious diseases such as malignant tumors; (3) a history of cognitive impairment or mental illness; (4) Participants in other research projects at the same time.
