研究目的：

1.本研究旨在检验本方案的可行性。 2.本研究旨在检验由护士主导的基于自我调节理论常识模型的心力衰竭自我护理项目对改善患者自我护理行为及健康状况的效果  

Objectives of Study：

1. The purpose of this study is to evaluate the feasibility and acceptability of this program. 2. The purpose of this study is to examine the effects of a nurse-led common-sense model of self-regulation-based HF self-care program on improving self-care behaviors and health outcomes.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 年龄≥18岁； 2. 符合心力衰竭诊断标准和按纽约心脏协会心功能I-IV分级； 3. 能够完成问卷填写； 4. 能够使用普通话或粤语进行正常沟通； 5. 能够使用微信或手机进行随访； 6. 同意参加本研究。

Inclusion criteria

1. Aged >=18 years; 2. Meet the diagnostic criteria for heart failure and the New York Heart Association heart function I-IV rating; 3. Able to complete the questionnaire; 4. Able to communicate normally in Mandarin or Cantonese; 5. Can use wechat or mobile phone for follow-up; 6. Agree to participate in the study.

排除标准：

1. 患有急性心力衰竭； 2. 患有认知障碍，如痴呆； 3. 患有精神疾病； 4. 患有其它严重疾病，如肾衰竭，癌症晚期等； 5. 近期参与了其它研究项目。

Exclusion criteria：

1. Acute heart failure; 2. Suffer from cognitive impairment, such as dementia; 3. Suffer from mental illness; 4. Suffering from other serious diseases, such as kidney failure, terminal cancer, etc.; 5. Recently participated in other research projects.
